Hello,

My first time to ask a question. I keep seeing people have used the I-693A vaccination supplement form. I recently married my fianacee, she came here on the K-1 visa, and had the full medical exam in Manila. I cannot find the supplement form anywhere. I have been looking for 2 nights now, spoke with 5 different people at the immigration help line with no success. Can someone help out there????? Thank you...........

There is no longer an I-693A form, unfortunately. It would make the CS transcription SO MUCH EASIER!!!

The vaccination supplement now consists of the regular I-693 Part 1 (about you) Part 2 #5 ONLY (vaccination section) and Part 5 (Civil surgeon certification). The rest of the fields can be left blank or have N/A written in.

Don't let your CS be sneaky like ours and sign part 6 instead. Gotta watch 'em! EVERYTHING they do!

K-1:

January 28, 2009: NOA1

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

You must use the I-693.

Fill in only Part 1 and DO NOT SIGN THE FORM until instucted to do so!!!!!!!

You will not write or fill in any other information on this form. It is for the Civil Surgeon to complete.

Do check to be sure the Civil Surgeon has completed it properly!
