These will all be specific to your consulate.

Please add your country and you will get more help from people from the same region.

One thing that is universal for almost all countries (certain ones are excluded from this list) is a police report that you need to obtain from every place you ever lived for longer than 6 months since the age of 16. By place I mean a country, not different cities. 1 report per country.

No, any country other than your own needs to be 12 months you have lived there as stated here: http://canberra.usembassy.gov/fiance_niv_appl.html under the topic "police certificates" in the middle or so of the page.

You will also need a medical, support papers and a few other forms.

Again, depends on a country.... Ukraine requires it for any country where you lived more than 6 months.

So since OP neglected to specify the country.... I'm just going by what I know.

Really? That's weird, because this link: http://www.law-visa-usa.com/fianceevisa.html says this "Police clearance(s) from all districts/countries where foreign fiancée lived since obtaining age of 16. Police certificates or clearances are required from all places where he/she lived for more than 6 months (if in her native country), and where he/she lived for more than 1 year (if in a foreign country)."

This one: http://www.ukrainefiancevisa.com/ says this "If an applicant has lived abroad for more than one year a police certificate must also be submitted from the country in which he/she lived."

They're all Ukraine sites. The US embassy in the Ukraine doesn't specify (at least in English) so if you have the link that'd be good :)

I don't have a link, but it is in their "packet" that is downloaded off the consulate website.

INSTRUCTIONS FOR IMMIGRANT/K VISA APPLICANTS:

· 3. POLICE CERTIFICATES:

Applicants aged 16 years and older must submit a police certificate from the country of nationality and the country of residence at the time of visa application, if the applicant has resided there for six months or more. (For example, an 18 year old Russian citizen residing in Ukraine must present a police certificate both from Russia and Ukraine).

Police certificates are also required from all other countries where the immigrant visa applicant has resided for one year or more after the age of 16 including the republics of the former USSR. Fiancée visa applicants must present a police certificate from all countries where they resided for 6 months or more after the age of 16.

A police certificate must also be obtained from the police authorities of any place where the applicant has been arrested for any reason, regardless of how long he or she lived there. Police certificates must cover the entire period of the applicant's residence in any area. A certificate issued by the police authorities where the applicant now resides must be of recent date when presented to the consular officer. The term "police certificate" as used in this paragraph means a certification by appropriate police authorities stating what their records show concerning each applicant, including all arrests, the reasons for the arrests, and the disposition of each case of which there is a record. In Ukraine the certificate IP-1 must be obtained in the oblast centers from the Departments of Information Technology at the Ministry of Internal Affairs. The police certificate is valid for ONE YEAR.
