To do the medical exam, we were asked to bring proof of immunization. http://www.wokingmedical.com/Appointments.html

We got a note from our doctor basically saying "Immunization status up to date for MMR + Tetanus". Is that enough?

Our medical exam appointment was set to be very close to the interview, I want to make sure we don't make any mistakes that would cause any delay.

That would be Vancouver, Canada.

I'm actually interested in the answer to this as well as I'm planning on making a doctor's appointment this week for this very reason.

I believe you need the dates that the vaccinations were administered because the panel physician needs to record it on the immunization record form

I agree with kaffy. I presented a list of immunizations my daughter had since she was a baby, how many she had (some had to be given in 2 or 3 interval periods) and what dates they were given.

You need either the dates the vaccines were given (that yellow card that you get when you get vaccinated) or the results of your titres test.

Dr. Seidan's office in Toronto has this to say on their website, but all Canadian immigration medicals require the same documentation at the medical

How do I bring written proof of my immunizations?

You must provide the panel physician with a written record of your vaccine history. Ideally the written proof should list each dose of each vaccine you have been given -- the date (month, day, year) and who gave you the vaccine. You cannot simply tell the panel physician that you had vaccinations as a child. We cannot accept notes from your doctor like: "Vaccinations up to date" or "Does not require additional vaccines."

When no written vaccination history is available, you may present the Panel Physician with written laboratory proof of immunity in the form of antibody titers. A titer report should be dated and either be on your doctor's letterhead or be a copy of the actual laboratory report. Your personal physician can arrange the necessary blood tests. However, depending upon where you live, test results may not be available for a number of weeks. When no written vaccination history is available, but you can recall receiving immunization injections, we strongly recommend you discussing having antibody titres to determine whether additional immunizations are medically necessay.

Print of the list of vaccinations required by USA. Go to your doctor and go through it with them. They will give you the shots you need. Ask them to give you a print out all all vaccinations you have received throughout your life. Take this with you to the medical.
