Your fiancee needs to fill out I-134, sign it and send it to you. You do not need to sign it. You will need to take the form to the interview and submit it to the officer interviewing you. Don't forget to take the documents with you that show financial support, I-134 by itself will not be sufficient.

Here is a list of things you need to take with you to the interview:

1.

Valid passports for the beneficiary and any dependent children.

2.

Birth certificates for the beneficiary and any dependent children.

3.

Proof of termination of any prior marriages.

4.

A police certificate from the current place of residence of the beneficiary, as well as from any place or places of residence for 6 months or more since attaining the age of 16.

5.

Police certificate(s) for any dependent children over 16

6.

The Form I-134 – Affidavit of Support (notarized) and evidence of financial support:

Bank Statement ___________

Letter from employer ___________

Most recent tax return (copy) ___________

Copy of paycheck stub ___________

Original documents bearing the signatures and seals of the issuing authorities are required at the interview.

I hope this helps,

Good luck!!

You must do this and send it to her. You can download the I-134 and prepare it now. Have the supporting documents be as recent as possible before the interview.

 I believe the OP IS the US citizen. He will not receive anything from the consulate. The I-134 does not need to be notarized since about a year ago.

Moving thread to:

US Embassy and Consulate Discussion

You are almost there and now you have to deal with the embassy. This is the place to post your experiences or questions related to this last step before moving to the US. Topics relating to I-134's, packets sent from consulate and medical & police certificates should be posted here.

YMMV

The Petitioner fills in and signs and sends the I-134 plus other proof of income to the beneficiary.

Thank you, for your help; I have one more question can you tell me, what is inside package 3 what kind forms.

Whatever forms are rwequired by that consulate for that visa application. It varies somewhat by consulate. DS156, DS156k, DS230, DS157 are all common, though not all are used by all consulates, and some are used differently by consulates. Kiev does not use the DS230 and the DS157 is for males over 16 only...at Kiev. Other require it for all applicants. The I-134 is usually included also. Specific instructions for that consulate are the most valuable thing in the packet
