Hi all dear visa journeys

Thank u for care, i entred in US on K1 visa on June 12, 2009 , my visa expires on Sep 10, 2009. i need to do AOS before i got expire. so before i came i did a medical exam INCLUDING VACCINATIONS and i gave it to the embassy in my country ( it was a confidential envelope) that was asked as one of needed documents to get visa.. now i'm in USA and need to complete PART2 in form I-693, but i don't have any proof or documents about my vaccinations to take with me to the civil surgeon surgeon. so plz help me i'll be so greatfull specially because time is running out and i have only one week to send the file.

So plz can anyone help me about this

- can i go to the civil surgeon without any vaccination documents ?

- how much the vaccinations cost ?

- i'll pay 1010$ for the AOS forms and i'll send also the form 765 (work autho), do i have to pay extra fees for I-765 ?

- what I-94 number means ? is it the admission number on I-94 ?

plz my freinds i need urgent help and thank u so much

I don't know about the vaccinations. The $1010 includes the fees for I-765 Work Authorization and AP (I-131?) if you apply for them. Of course, you don't get a discount if you don't apply for them. I-94 number is listed on the I-94 in your passport.

Call a USCIS designated civil surgeon and find out what they require. Lots of people lose their DS 3025 at POE andseem to get by OK.

I-131 and I-765 are free if submtted with the I-485 $1010 fee.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

Did your doctor back home give you a separate photocopy of the DS-3025 when you were doing the K-1 visa?

The quickest way to deal with this is to re do the vaccinations at a public health department (most likely you can get them all except the varicella/chicken pox- the civil surgeon was the only person willing to give the vaccine to an adult when my hubby was trying to get his vaccinations). Getting the vaccines again will not harm you. Take the records from the public health dept with you to the civil surgeon, he or she will transcribe them to the I693A. Depending on the CD they may charge you and conduct of physical exam (not necessary if you K1 medical in your country was within the last 12 months), or just charge for filling the vaccination form.

If you plan to go to the Health Department, make sure you call and make appointment as soon as you can since they are always busy. It may take them days or more in order to schedule you. Health Dept in Charlotte has certain days to schedule new patients so your Health Dept may be the same. As for the cost, call them and they should be able to tell you. You can also tell them that you have taken the Varicella in your country and I think they are able to do bloodwork to test your vaccine in your system.

Just call them as soon as possible is my advice. Good luck.

Yes, they can check that you are immune to varicella by doing a test called a titer. It will take time to process (2-3 days at best) the titer. It is probably as expensive as the vaccine. Might be quicker to just get the jab. But ask lots of questions and make the best decision for your situation.

Call and ask, but they are usually pretty reasonable $10-$20, but the price varies by location. My hubby got his all of his vaccinations (except the varicella!) for under $20.
