My fiancee (now my wife) is here on a K-1 Visa. I got the rest of her vaccinations done over here. The civil surgeon left the name blank on the I-693-A form because we weren't sure whether to use her married name or her maiden name that appears on the medical chart from the Philippines. Should I start using her married name on everything now? Insurance...adjustment of status forms...etc? Thank you

If she has her ss card with her new name then go ahead, if not she has to change it in ss office so she can start using it. In the AOS paperwork they also ask for other names used so there you put the maiden if she is using the married name already.

Good luck!

Which name she chooses has NOTHING to do with her ss card.

She should use whichever name she wants to be known by. You should be consistant, always use the same name. Don't use married some places, maiden other places. In most states, a marriage certificate can be used kind of like a name change order. You don't have to "officially" change her last name with Social Security or anybody else.

For AOS (and medical form), she should use the name that she will use for the future. If she plans to use your name, write that on all the forms.
