Vaccination record for AOS

Can anyone tell me if a US department of State Vaccination Documentation worksheet (form DS-3025) and an international certificate of vaccination (China) is enough to send in with my AOS? If not, would this be the same documentation that I would provide to a US Civil Surgeon?

you would need to transcribe your records onto a form called I-693, goto any civil surgeon to get that done, you can goto USCIS.org to look for a civil surgeon near your area.

The instructions say to include form I-693 with your AOS. DS-3025 may work, but most likely you would receive an RFE and it would delay your case.

Hi, I took a risk and submitted only my DS-3025. We did it because we didn't want to miss a deadline and we wanted to start our case being processed asap ( we just thought we will submit the other form if they ask later ). We were lucky and didn't get any RFE :-) But I was pretty stressed about this detail all the time ! I don't know what to advise, this was my case. Good luck!

We only submitted the DS-3025 (it was a certified copy with the consulate's stamp on it).

We will see if we get an RFE. It is dumb if we do.
