Problem with CSC RFE

I got my RFE from CSC today and it contained the blue letter and the white letter, but no form. I seem to remember from some other folks' posts that there was a special form, right? Did anyone else just get the two letters and no form?

You need the blue form and the white one with the questions on it.

Yes, this is the form we have been waiting for. If you can't find a copy online, someone could scan one and email it to you. I made a copy of mine, but I filled it out before I copied it.

Mine has a green one with NOA and all our information on it. The second one is the IMBRA questionaire and the last one is the i-129f supplemental informations. The first green one has back and front pages.

Mine was like that too. The other guy probably got rfe for something else other than IMBRA.

I also had one page with instructions and one page with the questions.

Yeah my first page was like that, too, but blue. I got the instructions page, but no questions :(
