Hello,

I am filling out all these forms that i need for the adjustment of status and I have a problem with the Medical Examination. I've already had it back in my country, Peru. I need a document that shows that i had all these vaccinations but i dont know exactly how does it look like? .. I dont know what is the name of the document. But i know that at my first arrival in USA . I gave this person a envelope medical examination and I just want to if it was in there . how can i get?

Because i dont want to pass through all that again.

Note K-Visas can defer vaccinations until the adjust status, more than likely vaccinations record is not in the immigrations packet turned in at the POE.

You need to keep any vaccinations record from home country separate and bring them to the USA and have them transferred to an I-693 by a US civil surgeon for inclusion into your I-485 AOS package.

Sometimes international vaccinations record is a yellow book.
