Just leave it as is, until you need a new one. And remember any time you fly anywhere make your reservations in your maiden name, because they can't let you on the plane if your passport name doesn't match the name on your ticket.

I can do that! I have become very skilled at being over-prepared when it comes to traveling lol.

I figured I would had myself to the thread.

I am ordering a new one. I found a guarantor, as well as two references. I did not have a picture ID, so I was told to send a copy of the SIN card showing my signature on the back, as well as a birth certificate.

I hope I get it fast! :star:

Adjustment of Status

11/03/10 ------- AoS (I-130/I-485) Package mailed out (Priority Mail)

11/07/10 ------- AoS Package received and singed for

11/10/10 ------- NOA1 received for I-130, I-485 and I-765 (emails)

11/12/10 ------- NOA1 received for I-130, I-485 and I-765 (hard copies)

11/12/10 ------- Touches on I-130, I-485 and I-765

11/19/10 ------- Biometrics appointment letter received

12/06/10 ------- RFE for I-693 (I think the issue is that it was not signed. Called USCIS and will receive a letter in a few days explaining)

12/13/10 ------- Biometrics done

12/16/10 ------- EAD card in production (email)

12/20/10 ------- Received "Letter of Explanation" for RFE (Service Request to expedite my case. Called USCIS and was told to ignore that and just send in response to RFE.)

12/22/10 ------- Touch (Email for Post Decision Activity on EAD saying that a letter of approval has been mailed out)

12/24/10 ------- Received EAD in the mail

12/27/10 ------- Applied for SSN

12/31/10 ------- Received Interview letter

01/03/11 ------- Received SSN card in the mail

Those of you that mailed out your passport from the US....did you receive all of your original documents back fine? They're asking for your birth certificate and all.

I sure did! They say they received their passports back, just wanted to be sure about marriage certificates and birth certificates ;)

I guess I'll add to this thread, my situation. Just when I'm getting ready to file all these problems with my paperwork popped up and the most pressing issue right now is my passport. I just realized yesterday when I set eyes on my birth certificate for the first time in twenty years that the place of birth on my passport is wrong! The passport lists the district instead of the village I was born in. Commence panic and scrambling as I called up passport services.

The woman on the other end was helpful but she sounded a little concerned that I had let this discrepancy go on for so long (been using my passport for a little under four years now). She advised me to fill out a new application and submit a letter explaining why I'm requesting a new passport. Then head down in person to a passport office. I hope I'm not in trouble. Has anyone been in this situation before?

It was a honest mistake because I hadn't even looked at my birth certificate until very recently (was not born in Canada by the way). Going to be heading to the passport office on Monday. Wish me luck. I will update on my situation in case anyone is interested in what happened.

You cannot use the simplified renewal form if you are changing your name, you will use the regular adult passport application:

They give instructions for name change :

NAME TO APPEAR IN PASSPORT

Write the name you want to appear in the passport. If this name is different from the name on your documentary evidence of citizenship, the original of the documentation listed below may be required:

– Marriage certificate

– Legal change of name (original document from issuing authority)

Additional documentation will be required to verify your identity as well as the name to appear in the passport (see instruction no. 5 "Documents to Support Identity").

SURNAME (LAST NAME) AT

Yes - you can use your mom - I used my brotehr - as long as she meets the qualifications:

Your guarantor:

must be a Canadian citizen 18 years of age or over;

must hold a five-year Canadian passport that is valid or has been expired for no more than one year on the day you submit your application;

must have been 16 years of age or older when he or she applied for the passport identified in section 2 of this application form;

must have known you personally for at least TWO (2) years*;

To know an applicant personally means that the guarantor is able to confirm aspects of the applicant's personal attributes such as name, approximate age, place of birth, physical description and some personal history such as occupation and place of residence.

must provide the requested information contained in his or her passport;

must be accessible to Passport Canada for verification;

must reside in Canada, in the USA or in any of the areas where submission of this application form is authorized, e.g. Bermuda. It is not necessary that the guarantor reside in the same country as the applicant.

A family member or person residing at the same address as you may be your guarantor, provided that he or she meets the specified requirements.

now, i'm under the impression that because i will be mailing the app from the U.S and because i reside in the U.S i must follow the old guarantor rules , meaning it has to be a person from the list of professionals ( Ie: dr, dentist, lawyer)and they still have to sign the back of one of my photos. Am i correct in assuming this? or can it just be a 5 year passport holder.

No - everyone follows the new guarantor rules - they're part of the form I linked :) yes they sign you photos, but no they don't have to be the silly old fashioned list of professionals
