It will depend on if the g325a is for the US citizen or for the intending immigrant spouse. If it is the one for the US citizen, you would put your spouses last name (family name) If the form is for the US citizen and the US citizen is the Wife, she would put her husbands family name. If the Form is for the US citizen and the US citizen is the husband then he would put his wifes last name but if she has taken the name of the husband, they want the last name she had before married (maiden name). Otherwise if you are not married you would place none in the space

Same will be true for the g325a needed for the intending immigrant. except the name data etc will be reverse

Thanks for everyone for your helping,in our situation the wife is a usa citizen,so what she does she will type in that question and what her husband will write when he fill his form?

On her form, she writes your name as her spouse.

On your form, you write her name as your spouse.

Her "maiden" name, is the one that is on her birth certificate.
