Hi guys,

I need some help on how to fill out the for I-693. I have the vaccination records from my country (Argentina), I'm wondering if I just need to send that and part 1 of the form or if I have to go to a civil surgeon and have him doing it. I'm afraid i don't have much time since my F-1 visa expires on January 3rd and I need to file before that after I'm getting married next monday.

You cannot just prepare the form yourself and submit it... the I-693 is not acceptable to the USCIS without a civil surgeon's signature...

You HAVE TO go to a civil surgeon.

He will fill the form and give it to you on a sealed envelope, which you add to your AOS package. USCIS accepts only sealed envelopes for this form.

the civil surgeon will prepare that form and the only thing you will do is sign it before he gives it back to you sealed...

4/14/12- 3rd yr as PR

1/17/12- mailed packet

Biometrics-- waived

4/25/12- interview- passed & took my oath the same day!!-- US Citizen!!!

My N400 Journey took 3months & 8days!:)

Ask for a copy before he seals it :thumbs:

Don't let the Civil Surgeon charge you too much. Shop around your area and see if there are other CS's that charge less. A full exam (with bloodwork and TST test, and form filled out) should cost no more than $200-250, if all your vaccines are up to date. But some people have reported paying $400+ which is crazy.
