My fiance & I will be getting married at the end of the month and I'm trying to get all the AOS forms filled out so we can send them in ASAP once we get back from Vegas.

In reading the guide for AOS, what is the I-693 form? (or the I-693A?)

My fiance had his medical the same day as his interview back in June & I don't recall them ever giving him a form back when we left. The Knightsbridge doctors needed more info from his GP, so when we got back from London to his town, he went straight to his GP and they sent out the extra info needed the next day. All they gave him was a copy of what they'd sent to the Knightsbridge docs.

He had all his vaccines done and got the paperwork from his GP stating he had chicken pox when he was a child.

I don't want him to have to unnecessarily have to go through more vaccines & blood work.

Hopefully someone here can fill me in on this... I'm at work now so I can't look and see what papers he has in the folder we made up for his medical exam while in the UK, but will do so when I get home.

Do you have records of him having received all required vaccinations? Either from GP or immigration medical (DS-3025)? If so, youcan have a Civil Surgeon transcribe the information on to the I-693 Part II Item 5

FWIW, the I-693A is now obsolete (as of May 2008)

At AOS my wife had the same thing, basically a vaccination card from her GP in her home country. We took it to a local civil surgeon who (for $50, cash only, the jerk-immigration is a RACKET) transferred it to the official USCIS vaccination supplemental form and gave us the supplemental form in a sealed envelope. We attached the sealed envelope to the rest of the I-485 package and sent it in.

When we went to AOS interview the interviewer had both the medical data (turned in at POE) from the original medical evaluation for the embassy interview and the vaccination supplemental form that we sent in. The chicken pox vaccine was not checked on the supplemental form, so he went to the embassy medical evaluation where the civil surgeon there had checked it off that she had chicken pox in early childhood (I think it was age 3 or 4). He made a note on the supplemental form where that vaccine was supposed to have been checked and approved our I485.

I will look and double check all the stuff from the medical that we saved in a folder when i get home from work, though i can't recall ever seeing anything that said DS-3025 on it

Here's some info for you in the process.

Here's the USCIS document on the new vaccine/TB skin test requirements. Depending on when the medical was done, you may or may not need something new...

Do not let the doctor try to get you to do more than you need, a new medical is not needed. It says so clearly in the instructions for the I-693. David and I paid $25 for the transcription. Call around to different Civil Surgeons or use Dr. Arnold on the first page of the I-693 info (see link below).

AOS & getting the SSN & the I-693

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

8/11/08 NOA1 for AOS, EAD and AP; 9/2/08 Transfered to CSC; 9/4/08 Biometrics

10/24/08 Rec'd AP; 10/27/08 Rec'd EAD; 11/24/08 Rec'd GC
