DS-3025 for AOS

Hi,

I got the form DS-3025, for vaccinations, from the medical examination in Mexico. The form is empty, except for the doctor's signature, can I send this one with my AOS documents? Or does it need to be filled out by a doctor?

You have to finish the I693 form:

Please notice, you only need to finish the portion about vaccinations if you have your medical examination done for your KI, not long ago.

The DS-3025 is not what you're supposed to send in with AOS. You're supposed to send in an I-693 form, filled out, and signed by a USCIS Civil Surgeon. It can't just be any doctor you choose.

The DS-3025 is a vaccination worksheet that is given to applicants at their overseas medical exam. It's supposed to show what vaccinations they received. If it is blank, then you may have a problem. You will either need to prove in some way which, if any, vaccinations you already received, or you'll have to repeat the ones that are appropriate for your age.

An empty form does you no good... For AOS you need to provide evidence of required vaccinations... Unfortunately all your evidence indicates now is "nothing".

Do not let the doctor try to get you to do more than you need, a new medical is not needed if you've had one in the last year. It says so clearly in the instructions for the I-693. David and I paid $25 for the transcription. Call around to different Civil Surgeons or use Dr. Arnold on the first page of the I-693 info (see link below).
