You can get the vaccines anywhere you want, but a USCIS approved civil surgeon has to complete the form certifying that all required vaccinations are done.

before her interview at the USEM. I see where she will need to have these vaccines before filing the AOS? If so.. where and how do I go about getting these vaccines for her and what forms do I need?

you can locate a civil surgeon here: https://egov.uscis.gov/crisgwi/go?action=of...office_type=CIV and let them know what you need, each doctor has there own fee...take the form I-693 with you when you go to visit them and they will fill it out...

Oops just noticed you said her interview at Usem...I'm not sure about that but i think she has to visit her local physician (in home country if that's where she is) and let them know...uncertain if they use the same form, hopefully someone can shed better light on it than me...goodluck

Bummer, they were included for free in the medical fee in manila. now they will cost you a few hundred if she skipped them all. Normally you only need to get the TB shot which you can find for free at clinics in alot of cities. You can download the form from the internet (I-693) that she will need and the above list of Civil surgeons will get you someone to give the shots/fill out the form. You have to have a civil surgeon fill out the form. Any old doctor wont do. They will fill out the form and then give it to you sealed. you will include that with your AOS.

Yes... My wife didn't compute the cost when she turned them down :( But... they needs to be done. I have looked and I can't find a list of what she needs. I called the local health department and they do give quite a few.. but the lady told me I need to know which ones are required. Does anyone have the list of required vaccines

From I-693 instructions:

By law, the required vaccines include: mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, influenza, hepatitis B, haemophilus influenzae type B, varicella, pneumococcal, rotavirus, hepatitis A, meningococcal, human papillomavirus, zoster, and any other vaccinations recommended by the Advisory Committee for Immunization Practices (ACIP).

Many of the ones from the list above will not apply because they are gender and/or age-specific.

I'm pretty sure this is the same schedule the Civil Surgeons use to determine who needs which vaccine.

You can't skip the one part of the CS thing if you have had recent required vaccinations to inform the USCIS about. Only the CS can prepare the required docuemntation to submit

We did not even send in a I-693, my wife was pregnant, and she got her Green-card.

We just sent in a copy of medical she got SLEC.

Not recommending it, but I wasn't going to let them inject that ####### into the bloodstream of my unborn!

Has anyone been RFE’d for missing it?

They will excuse the needed vaccines if you are preggy and have a letter from the doctor stating its not recommended at this time. You just include the letter with the vaccine supplement you got in manila. However the original poster didnt say they were pregnant, just that his wife had declined the vaccines. If you are not preggy then they are required. They will issue a RFE for them.

She may not need to take any at all. If she has her vaccination record from the phillipines, she can just take it to a civil surgeon to have them transcribed in to I-693A. If she is missing any then she can just get them.

My husband didn't take any vaccines for his interview, but he did bring his vaccination card to the US (the one he's had since he was a child) and he had them all...we only paid 20 dollars to have form I-693A completed.
