Hi -

Today my fiance has his medical appt with the civil surgeon for the AOS. He should only need the I-693 supplement because he already had his medical for the K-1, but I don't know what he needs to bring to the appointment to prove his vaccinations. I've read about having a copy of the DS-3025 but I don't believe he has that. What he does have is a million different signed records for each of his vaccines - but they're in Spanish. What do we do?

Thank

Liz

He will need the vaccination record translated into English... If he doesn't the Civil Surgeon will not understand it (unless the CS understands Spanish?). But if the doctor doesn't understand his vaccination record then your fiance will be told which vaccines he needs to get to meet the requirements.

What he needs to bring to the appt is his checkbook or a bag of cash because if he can't prove that he had the vaccines done, he'll be paying $20-50 each for all 10-15 of them. $$$$$$$

Have you discussed with the doctor that he only needs the vaccination supplement, not a full medical? If you just made an appt with the CS for an AOS medical, there may be an issue when it comes to paying the bill. They will charge you a couple hundred for the full medical. A lot of these doctors are real slick and try to convince you that you need a full medical.

The best "reason" (lie) I heard was that the medical was performed outside the US so it's no good for AOS.

Thankfully my mom saved all of my medical cards from since I was a baby so

I brought all of those the day of medical appt it helped me alot cause it was solid proof. The only thing

I had to pay extra for was the tetanus shot(which he said should be done every 10 years) so I was overdue for one.

If the medical doctor doesn't have the necessary documents(dates) on hand that proves your vaccination history.

You will have to pay additional amount for each shot required for AOS and the cost does adds up.
