Timeline

I know my wife doesn't need her immunizations or anything because she had them done in Bangkok for her K-1 Visa...but do I need to fill out any form stating this or do I just mail in a copy of the immunizations/vaccinations from the hospital?

24 JUL 07 : K1 arrived in Lincoln, NE

24 JUL 07 : USPS status of package: DELIVERED:)

02 AUG 07 : NOA1 from CSC

14 DEC 07: NVC sends to Bangkok

17 DEC 07: Consulate receives

08 JAN 08: Packet 3 sent

08 JAN 08: Given date of interview

11 JAN 08: Interview...approved already before getting to the window!...NO questions asked

25 JAN 08: My lady arrives in IAD-Washington DC:)

19 APR 08: Married in Alexandria, VA (5 days before Visa expires)

20 JUN 08: Mailing in AOS packet

18 DEC 09: GREEN CARD RECEIVED:)

Since a K-1 technically has no required vaccinations (they are only strongly suggested), the question is what information was included in her medical report. I do not think you can definetively say that was is a part of the report. It may have been or it may not have been. Did the clinic provide to her a copy of the DS-3025?

Do a search on here for Dr L E Arnold, you can do the I693A all by mail for like a $15-$20 charge! Good luck. :star:

Send in that by mail to Dr L E Arnold, make a copy of it for your records before you mail it to her. They do I693A forms based off information from your DS-3025. :thumbs:

Contrary to popular belief, many people have been very successful submitting the DS-3025 instead of paying money to have a CS transcribe the DS-3025 to the I-693 Part II (formerly referred to as the I-693A).

Can I just send the DS-3025? Her visa alreayd expired 24APR and we havent applied for AOS yet....i really dont want to wait any longer and cause a problem.

I thought the DS-3025 was enough.

Another thing, on her DS-3025...the "Insuffiecient Time Interval" box is checked for all the vaccinations she was given.

Does this mean anything bad? The date she was given the vaccine is given, but to the right side of the form "Insufficient Time Interval" is checked.

Can I just send the DS-3025? Her visa alreayd expired 24APR and we havent applied for AOS yet....i really dont want to wait any longer and cause a problem.

Depends on whether it is a required vaccination for AOS purposes... If the series is not complete but it is required then you need to get it completed.

PS. I only had one page of the DS-3025

Sounds like her vaccinations may not have been completed. She may need to visit a civil surgeon. Call Dr Arnold's office and ask them first. That's your cheapest route. See the pinned topic at the top of this forum for the contact information.

This is for my own knowledge. How long after the first varicella shot are you supposed to get the second?

Since the USCIS bases the required vaccinations from the CDC, the CDC is your source for this answer. Accordingly, the CDC says at least 3 months or 4-8 weeks.

All healthy children 12 months through 12 years of age should have two doses of chickenpox vaccine, administered at least 3 months apart. Children who have evidence of immunity to varicella do not need the vaccine.

People 13 years of age and older who do not have evidence of immunity should get two doses of the vaccine 4 to 8 weeks apart.

I am currently preparing AOS for Rin and her children. Rin's vaccinations were complete. The children's DS-3025 had checks in the "insufficient interval" box. As Rin has been here over a year (K-3 visa), she had to go for another medical. The civil surgeon also filled out the forms for the children after we went and got their second doses of MMR, varicella, etc. Cost for them for the CS to complete the I-693 was $40.

Also, I-693a no longer exists (as someone mentioned before), the instructions for the I-485 state to submit I-693. I-693 instructions say to submit parts 1, 2, and 5 if a medical has been performed in the past year. To be safe, I would find a civil surgeon to complete the I-693. I know others have gotten by with the DS-3025, but the new I-693 with the new instructions was issued in April 2008. I would not assume that a DS-3025 is still being accepted.

edit: I just looked at Rin's DS-3025. Her form was either checked as "complete" or "not age appropriate". No checks in "insufficient interval".

I would not assume that a DS-3025 is still being accepted.

Slow down, you cannot assume the latter either..... there was someone within the last few days who received and RFE from the USCIS specifically requesting the DS-3025 as evidence of vaccination.
