Local Civil Surgeon refuses to provide I693A

Not necessarily. My wife's DS-3025 was accepted as suitable evidence that all required vaccinations were either had or waivable as medically inappropriate. Not everyone has this experience, but my wife is far from unique.

An important bit of information that summersolstice hasn't provided is whether or not the DS-3205 was complete. If it is not then there is no way to avoid a visit to a Civil Surgeon.

just keep looking.... they want to take advantage of us,because they know we do anything to have papers ready.... in my case i didn't send the supplementary form i send the form that they gave my husband at the american embassy, we haven't receive a letter yet saying they need the suplementary forms... i wil way and see what happens....

You will either get a RFE or need to have it for AOS interview.

Using the convenient physician locator, I was able to find a single local provider here in my community. Unfortunately, when I contacted his office, I was advised that "the doctor only provides full physicals" and they refused to provide an I-693A. I couldn't believe the response and asked for an explanation. I was told that the doctor places his signature on the form and will only do so when he has personally completed the entire physical (for the full fee, of course!). Well, Plan B now requires us to make an 90-mile round trip to the next nearest doctor...

i just want to share my experienced. When I had my td shot and SUPPLEMENTAL FORM TO 1-693 done the doctor explained to me the difference between SUPPLEMENTAL FORM TO I-693 AND I-693A.

SUPPLEMENTAL FORM TO 1-693= is the transfer of record from US department of state DS 2053 ( Vaccination Documantation Worksheet from St. Lukes ) to this form. (AOS applicant's Documentation Immunization)

I-693A= is a required whole MEDICAL EXAMS.

It's true if you ask them to provide you the I-693 that's mean that you have to take the full medical exam, instead explain to them that you need only the "supplemental immunization form I-693A and whatever shots you took they will transfer to that supplemental form.

that is the exact response i got when calling for my husband. they also got very rude and said that I needed to be resonsible enough that i needed to bring the form in with me (I-693A). then i was able to contact another one and they told me that i was probably right. and that the government does not keep them up to date. but, they too said that if they are to put there name on a form they needed to do the whole exam themselves. i took the chance and we only sent in the DS-3025. and thank the lord up above we flew through the AOS process!!! best of luck!!

the DS-3025 worked just great for us. we took the chance on getting a RFE and we didn't receive one. i figure y would the U.S allow you in if it wasnt ok to begin with?? best of luck!!!

Perhaps some of the confusion may be coming from a Dr. being asked to transfer another Dr's information? I may be wrong on this but our Civil Surgeon had to be "convinced" by us taking all the evidence (medical records and inoculation records) to him. He wouldn't even LOOK at the work sheet. Wanted to see the proof for himself (which I understand). Also wondered about wording it as "I693A".. the Receptionist/staff may tune out the "A" bit and translate it as "medical".. Perhaps a Medical Supplement form is the better way to state it? I dunno.. it took some communication to explain to our Civil Surgeon WHAT we wanted.. cause he was totally unfamiliar with the I693A form. They mostly do the full medicals.. My 2 cents worth.

That may be because there is no such form as I-693A. What you are talking about is actually called "Appendix A, supplement form to I-693".

They would allow you in because the K visas are non-immigrant visas, and non-immigrants are not required to have vaccinations.

We had a doc that was a 15 minute walk away, so naturally we called them first. Being near Philly, we had a few to choose from. They told us they only did aa full medical. My wife explained she only needed the I-693A and explained what they had to do. They finally allowed her to come in but if it wasn't right, we couldn't come back. Full medical would've been $25, they said they'd take her for $75 for what she needed. We didn't have the form from immigration, only faxes from the doctors she had blood work at. We took the instructions from the I-485 saying what we needed, just in case. The receptionist filled it all out and then the doctor saw my wife. He just signed the form and off we went. And they only charged us $50. More aggrivation than we needed, but hey.

Oh, we also took a blank form with us.

We had the same problem as the OP. A doctor nearby told us that my husband had to have the physical done again ($) although I read from the USCIS website that he only needed the I-693A supplement.

I talked to a number of secretaries, a nurse, and office manager and they didn't want to allow it.

We found a doctor in Kissimmee (off 192 on John Young Pkwy) who transferred the vaccines in minutes and only cost us $25. It was a good experience.

Anyone from the Orlando area - just ask and I can give you the doctor's name.

After some convincing that it was okay to JUST give us the vaccination supplement, they charged us a pretty $75. Not to mention they were rude to us in the office.

Where did you guys go for that? We called IU Health Center and they wouldn't do it for us until after our marriage. Then we called some kinda holistic practitioner lady whose receptionist had no idea what the vaccination supplement was and argued with me about it in a very silly and rude manner until she was finally convinced I knew what I was talking about. After putting me on hold to consult with the doctor, she said it would cost $75.

We ended up getting Pigu's supplement filled in by a physician in Indianapolis for $20. The guy up there knew exactly what we needed.

Yup! We went to the crazy holistic health lady. She SWORE she couldn't do this for us. And then we read the part of the form where she said it could, and she said it'd be $75. So we just sucked it up and went. They were all so strange. Seems kinda two-faced to me, preaching about centering yourself and being one with the earth....and then charging an immigrant who isn't allowed to work $75 to transfer some checkmarks. We called and asked if we needed an appointment, she said no, just come in anytime and that it'll take 5 minutes. So I take time off of work to go in (because their hours are like 10-12, 1:30-4, Tues-Thur). We get there, and the rude receptionist says, "Oh, we can't get that in today. And the whole office is going on vacation next week, so it won't happen until next week." But I turned on the fake waterworks and told her that his visa was expiring and if we didn't get this in he'd be deported and I'd be without my husband. Blah, blah blah. They did it. :)

End rant.

Good move on heading to Indy. I wish I would have thought of that. $20 plus $20 in gas would have been cheaper!

I didn't have any problems getting the I-693A. I called 2 doctors in Richmond, VA. The first one, the "lady that does the pricing" was on holidays so I was advised to call back. The second doctor I left a message and he called me back. I told him all I needed was a vaccination supplement filled out because I came on a K visa and I had my records with me from my foreign medical. He quoted me $45 and I went in the next morning. His receptionist transferred the info from my DS vaccination worksheet, had him sign in, sealed it and gave me a copy. I had brought records of my immunizations from my doctor as well as the DS form and an unsealed copy of my entire medical but they didn't need to see it. I even brought a blank copy of the I-693A but they had them on hand and it seemed like they've done plenty of them before.

For anyone in Richmond, VA, I went to Dr. Patel on Monument Avenue. Totally painless and took 5 minutes from walking through the door to sealed vaccination supplement in hand.
