When I filled it out, I just checked "Born in the US" and left the rest blank. I thought the #, date, and place of issue were for Naturalized citizens. We didn't have any problems.

It's best not to leave things blank. N/A would be preferrable, IMO.

I had similar question. I was naturalized, but I think they took my certificate when I was applying for passport. I wonder if passport is enough. It's been so long, not sure if I still have the certificate.

It's enough but your answer would be "unknown" instead of "N/A".

Generally they mail your certificate back around the same time when they mail back your passport. You could get a duplicate copy, but I do not know the costs for it. However, almost all places accept the US Passport as proof of citizenship.
