AOS I693 form

Timeline

My finacee had the exam before the interview as is necessary and now we are in the AOS stage and I am not sure about

the I693 form.....do we have to go to another doctor here and take the form he was given (a one page vaccivnation documentation worksheet

from Canadian doctor...the form says at the top...for Canadian use only)

and have a US doctor fill in the I693 form?

Any information would be helpful???

May 5-sent I129f to Texas and they forwarded to CSC

May 11-priority date according to NOA1 on Hard Copy

May 14-received by CSC...notice sent (never received)

May 15-money order cashed

July 19-called about Hard copy..said they would resend

July 23-lawyer calls and is told application is lost and to resend application

July 25-sent a duplicate application to Texas and it is forwarded to VSC

July 27-called again...said they would resend from CSC

July 31/aug.3/aug.6 "touches"

Aug. 16 received the NOA1 from VSC for duplicate application

Aug. 16-called about Hard Copy...said that they would resend (3rd time) from CSC

Aug. 17-"touch"

Aug. 19-FINALLY RECEIVED THE NOA1 HARD COPY for original application from CSC

October-19-CRIS sent message that they were moving our application to VSC?!?!?

(seems USCIS sent our first application to the wrong center)

November-16 origianl is at the VSC

Dec. 3-congressmans office calls VSC...will be processed by July application date

Feb. 6-APPROVED!!!!

Feb. 7 sent to NVC

Feb. 13 sent to MONTREAL...WRONG EMBASSY!!!!!!! SUPPOSED TO BE VANCOUVER!!!!!!!

Feb. 14 arrived in MONTREAL

Feb. 20 talked to NVC center....supervisor will try to "catch" the application call back in 10-15 days

Feb. 29...talked to the State Department (Visa)...they had NVC send cable today to Montreal instructing them to send packet to VanCouver

March 4...K visa application received at VanCouver

March 6 packet 3 ARRIVED

March 10 packet 3 checklist and DS230 faxed back

March 12 medical exam

March 20 packet 4 received

April 8....interview date

From what I remember, as long as you had the medical done within the last year, no new medical is required. When I sent in my AOS pkg for me and my son, it was 10 months since I had the medicals done. When assembling the AOS pkg, I included a copy of both mine and sons vaccination cards and a copy of 1 sheet where my son needed an additional vaccination, which we got and I had my doctor sign that page. Immigration has a copy of your medical exam with them, so theres no need to worry about that. I hope this helps.

10/24/2007 - INTERVIEW DATE!!!!!....APPROVED!!

10/30/2007 - Visa in hand!

AOS Timeline:

1/30/08 - Sent AOS pkg Priority Mail with USPS

3/12/08 - Biometrics taken

3/29/08 - EAD and AP in hand for me and my son

5/21/08 - Email-GC on its way!...RFE for my son's application

5/27/08 - GC arrives!!!

7/02/08 - Sons GC arrives!!!

ROC TIMELINE

3/23/10 - ROC sent!

3/29/10 - NOA1

4/22/10 - Biometrics Appt for me and my son

6/30/10 - Email saying card production ordered for me and my son

7/6/10 - Letter in the mail saying cards will arrive within 60 days

7/10/10 - Both cards received!

You can take the vaccination record to a listed doctor on the USCIS website, and pay through the nose to have them transfer the info. Some will tell you that you need to take another exam. Which is NOT the case.

You can send it to the doctor in Texas(?) who will do the transfer for you for I believe $10.

Or you can take a gamble and just send a copy of the vaccination record from the medical instead of the I693A like I did and still get approved.

BUT it is a gamble, you may get an RFE for the I693A.

Are all of his vaccinations complete and up to date? Does it say that on the vaccination worksheet? If so, you could try just including a copy of the vaccination worksheet in with the AOS application. It will probably be accepted as is. If he doesn't have all of his vaccinations, then he will have to get the missing vaccinations and then take the worksheet and proof of the missing vaccinations being done to a US Civil Surgeon - there is a list of recognized physicians on the USCIS website. The Civil Surgeon will transfer the information from the worksheet and the updated vaccination records to a vaccination supplement sheet, put it in a sealed envelope and give you a photocopy (make sure you request one if he doesn't). It can cost anywhere from around $25 to $50. Many Civil Surgeons will try to tell you that you need a full medical - you don't. If they won't do just the transfer of information to the supplement, find another Civil Surgeon who will. There is a Texas Civil Surgeon who is willing to do the information via mail and he charges only $10 last I heard.

You don't have to include the supplement with the original application, although you are supposed to have it available by the interview - if you have an interview. As I said earlier, if the vaccination worksheet shows that his vaccinations are complete, I would just include that instead of getting the supplement done at this point in time. That is what I did and I was approved without an RFE and without an interview. Hehehe, I did go ahead and get the Vaccination Supplement done planning to bring it to the interview - I still have it sitting in my files.
