If you read the instructions that come with the form you will see that you do need to provide proof of the validity of your marriage with the application form. Examples of 'proof' are provided on the instructions. It is a good idea to make sure you have evidence that covers the whole span of your marriage and fills the categories suggested in the instructions. The link includes both the form and the instructions. Download them both, read them over, and if you have any questions after that come on back and ask them.

If you have changed address within 2 years, have to include a letter with old/new addresses. Plus, cover page should show a list of your evidence (with short description). Scroll this forum back-some people (like Kathryne41) give really great instructions how to prepare folder with evidence (like inserts on the botom, etc)

This is what we sent to USCIS. Make sure the material covers the span since the time they provided you the conditional card. I used tabs at the bottom of each section identifying the document section, used ACCO fasterner on top, put my check inside a little plastic bag and attached a few photos inside a small manila enveloped. All in one package. It may be over board but trust me, the less work they have to do looking for material and the easier you make it for them to sort through documentation, the faster you get your extension letter:

This is what we sent:

• I-751 application with attached check for $545.00.

• Copies of our apartment lease (2007-2008) and (2008-2009).

• Copies of car insurance notices (2006-2008).

• Three sworn affidavits by U.S. citizens. One from friend, two from family members attesting to our relationship and marriage.

• A copy of our joint US Federal tax returns and W2’s (years 2005, 2006 and 2007).

• Copies of our driver licenses.

• Copies of checking account check and three checking account statements (years 2006, 2007 and 2008).

• Copy of wife’s two year green card.

• Copy of wife’s passport (maiden name).

• Copies of wife’s Macy’s credit card statements.

• Copy of 401K beneficiary statement.

• Copy of our health insurance cards.

• Copies of itineraries/tickets about trips taken together.

• Copies of junk mail.

• Copies of family/friends’ invitations to events together.

• Copies of Community College registration bills (2006-2008).

• Envelope with pictures with family and friends.

01-19-2006 - Biometrics done in Brooklyn

01-19-2006 - 4th "Touched" for EAD

01-21-2006 - 5th "Touched" for EAD

01-23-2006 - EAD Approved !!

01-28-2006 - EAD Card received!!

02-15-2006 - AOS Interview Letter received -

06-07-2006 - Interview date

06-07-2006 -APPROVED!!!

07-17-2006 - Received Welcome letter. Now waiting for GC.

07-24-2006 - GREEN CARD IN OUR HANDS!! We are done for now!!
