BOTH, 1040 is just a form, but without supporting Proof like (W2 and/or 1099) the numbers on the 1040 mean nothing.

Better yet use IRS transcripts, they are FREE.

in the form question #6 i would select form 1040(tax transcripts)..but there is also question #8 about the w2...do i select that also?

Forget about the form.

Just call IRS order simple Transcripts, they will contain everything you need.

2007 wont be available, you will need to send a photo copy of (1040+W2 and/or 1099) for 2007.

The availability of transcripts is a function of how long ago you filed. I don't know how long after the filing date they become available but clearly many are not available because the returns haven't been filed yet.
