Question here:

My other half did not receive a TD shot in the Philippines when he did his medical examination. They said just get the TD shot here in the US. My question is, can we just submitt his DS-3025(vacination record) without the TD shot in supplement for I-693A or we need to have his TD shot then go to civil surgeon for us to have the I-693..I am really confuse on this part :blink: . Thanks and I appreciate the response! :thumbs:

You need to get a TD shot here in the US and have all your medical records (including the DS-3025 from st. lukes) to be transcribed by a civil surgeon to the I-693A. I paid $35 for it and got the shot for free at Kaiser.

Just thought it would be great to start with the thread for all AOS March filers. We will submit our AOS application on Monday (March3). So, with all the rest who are filing this March come and sign up! Good luck to all MARCH FILERS!

Hello there! I just joined this forum and I also just posted about my personal experience on the forum.

Anyways just to let you guys know my hubby and I posted the AOS package on the 29th Feb via Fed Ex and it arrived in Chicago on Monday the 3rd March. My checks were cashed on the 7th March.

This is what we mailed for anyone out there needing some extra input (hope it helps):

Petition for Alien Relative:

• Form I-130, Petition for Alien Relative, with $355 check;

• Form G-325A, Biographic Information, for the petitioner with 1 photo of the beneficiary/applicant and 1

photo of the petitioner attached;

• Form G-325A, Biographic Information, for the beneficiary/applicant with 1 photo of the beneficiary/applicant

and 1 photo of the petitioner;

• Copy of marriage certificate;

• Copy of beneficiary/applicant’s divorce certificates;

• Copy of petitioner’s birth certificate;

• Copy of petitioner’s [California] Driver’s License;

• Copy of beneficiary/applicant’s birth certificate (plus a copy of the abridged birth certificate translation);

• Copy of biographic page of beneficiary/applicant’s current passport; and,

• Copy of beneficiary/applicant’s I-94 card under Visa Waiver Program [both sides].

Application to Adjust to Permanent Resident:

• Form I-485, Application to Adjust Status, with 2 photos attached, with $930 check in payment of the I-485

and $80 for biometrics;

• Copy of marriage certificate;

• Copy of beneficiary/applicant’s birth certificate (plus a copy of the abridged birth certificate translation);

• Copy of biographic page of beneficiary/applicant’s current passport;

• Copy of beneficiary/applicant’s I-94 card;

• Form I-693, Medical Examination, in a sealed envelope;

• Form I-864EZ, Affidavit of Support, submitted by petitioner;

-Copy of sponsor’s US birth certificate;

-Copy of sponsor’s 2007 Income Tax Return and W-2

-Copy of 2006 and 2005 sponsor’s W-2;

-Last 6 months pay-stubs; and,

-Letter of Employment Verification of petitioner.

Application for Employment Authorization:

• Form I-765, Application for Employment Authorization, with 2 photos attached;

• Copy of biographic page of beneficiary/applicant’s current passport; and,

• Copy of beneficiary/applicant’s I-94 card [both sides].

Application for Travel Document:

• Form I-131, with 2 photos attached;

• Copy of biographic page of beneficiary/applicant’s current passport; and,

• Copy of beneficiary/applicant’s I-94 card [both sides].

Bona Fide Documents:

• Joint Lease;

• Bank statements of the petitioner and beneficiary/applicant;

• Petitioner and beneficiary/applicant’s joint account credit cards;

• Petitioner and beneficiary/applicant’s cellphone account information;

• Petitioner’s sales receipt in the form of an e-mail showing flight ticket purchase for beneficiary/applicant

(copy of credit card used);

• Boarding passes;

• Correspondence from various sources showing that the petitioner and beneficiary/applicant live at

the same address;

• Written affidavits from third parties;

• A selection of letters demonstrating correspondence between the petitioner and beneficiary, and;

• Photographs of the petitioner and beneficiary/applicant.

There is a number, but it isn't the one you want, it doesn't relate to anything on the website. You have to wait till you get your NOAs :(

There is a number, but it isn't the one you want, it doesn't relate to anything on the website. You have to wait till you get your NOAs :(
