Hi

We are in the process of collecting all the paperwork to file for adjustment of status for my fiancee (will get married in a few days). The VJ AOS FAQ mention that we need to include a vaccination supplement I-693A with the package. When my fiancee had her medical for K1 visa, she was given a DS-3025 vaccination form by her doctor overseas, but not I-693A. Do we need to go to a USCIS-approved doctor here in the States and have him transfer all the information from DS-3025 to I-639A form, or can we just file a copy of DS-3025 from her overseas medical?

Yes - you'll need a civil surgeon (here in the US) to complete the I-693A for you (usually a small fee and can be done by mail). The civil surgeon will (usually) send/give you a sealed envelope (for USCIS) and an open copy for your records. Search forums for I-693A for more topics. :thumbs:

There are several threads on this topic daily. You can check those for additional feedback if you like.

I did not submit an I-693a.... just a copy of the DS-3025. (Approved..... no interview.... no RFE.) If your vaccinations were completed as part of your medical, and your medical is current (within one year) you don't need to provide an I-693a. That's just for K visas who did not complete their vaccinations as part of the K visa process.

Background: Some countries allow K non-immigrant visa holders to defer the vaccinations until after they arrive in the US, which is why they request the I-693A with the AOS submission. If the beneficiary's vaccinations were completed as part of the visa process, they are not required to submit the supplemental vaccination form because all that information is enclosed in the brown envelope handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission. This is not to say that someone processing the I-485 will not request the information in an RFE. Sometimes the medical gets "lost" during the transfer.

yes you would need to go to a civil Surgent to have the DS 3025 transverd to a ! 693a form

but be carefull some will try to convince you that you need another full medicial , wich is not true for K1

You do NOT usually need another medical exam after entry on the K-1 visa, in spite of what the directions

with the forms may tell you! Ignore the instructions regarding a medical exam after entry on a K-1 fiance

visa unless your local USCIS office advises you otherwise.

there is also a great doc in TX you can send your file too , I did it cost 10$ but dont have the adresse at hand right now , as soon as I find it will post it ( unless someone is faster and post it before me ;) )

Unfortunately all of the surgeons around my area (Michigan's thumb) insist on doing the full medical. :angry:
