Yes, I had to wait a while at DOS because they initially lost the sponsor views.

Yes, DOS will send you a letter with the favorable recommendation. Soon after, you should receive a letter from USCIS saying they have received your case and then you wait for approval.

I got the letter in a week or two if I remember correctly.

I'm not sure I can help with your other question. I applied for AOS (based on marriage to USC) without the approved waiver and I didn't have problems, not sure it works the same for H1B.

My job applied by the H1B once I got the DOS letter. I had to scan the letter and send it to them. A few week after a got the DOS letter, I got the USCIS NOA 1 from the waiver. 

 

My job sent  the H1B premium processing 2 days after I got the DOS letter (with the letter attached). I got the H1B visa in 10 days and USCIS made the decision for the waiver the day they got the H1B petition. 

 

Hope that helps. I did AOS from F1 to H1B within the US.

Interesting. My job told me it will take 4 to 6 weeks for them. I am employed by an university. They said there are a lot of paperwork that needs to be done.

Yes, I am employed by an university too. But they paid the premium processing fee (~$1,500). If you were supposed to start in August, maybe it makes sense for you to pay the fee. I mean, if you can get the visa in 15 days that means you can start getting paid day 16. Otherwise, you will start getting paid in a month or more. So when you discount the fee you would still come on top in terms of payment. 

 

Just an idea. My department paid for the fee so either your department does not know they can do that, or they didn't want to pay it or don't have the funds. But you can ask them and otherwise, propose you can pay it.
