No, it is a document that they give you at the embassy, the same one that they give you if you are approved.  It is a general form that they can list when you are missing things and need to return to the embassy, if you are approved and when you need to return to pick up your passport with visa or if you are denied.  Her form only says denied, I believe, but at the end of the interview when they told her that her visa was denied the woman said that her reason for visiting the US was not sufficient. My husbands aunt has petitioned for her mom and her sister and both initial visas for both (applied for years apart) were both denied and in both cases she was told to wait a year before reapplying.  When she reapplied her mother was approved.  When she reapplied for her sister she was denied and it was stamped that she an attempt to reapply before 5 years would be an immediate denial. It isn't the Cuban government that is denying them, it is the US.

I've also never heard of anyone being denied for not having enough reasons to go to the US, it's usually the other way around.

Some countries have waiting periods. Costa Rica is 6 months post denial on a tourist visa. 2nd denial is a year.
