Confused on DS-3025 & I-639...

1) Here is the background...

07/25/2014 - Fiancee has medical exam and receives 2 shots

08/14/2014 - Fiancee has interview (approved) and turned in the sealed medical results

2) We are pondering whether to finish her 2 more shots in China or in USA

09/04/2014 - First day she can complete remaining 2 shots

3) Based explanations from (KayDeeCee, Nich-Nick, and others), if DS-3025 is completely filled out

then we can bypass I-693 (civil surgeon's transcribe) and just submit DS-3025 when we file AOS.

4) But, as of now, fiancee hasn't seen a copy of DS-3025, how did you get your's?.

Is this what's inside the sealed envelope that she turned into embassy during the interview?

Should she have gotten a copy of it after medical exam?

5) When embassy sends the passport & K1 visa, do they also include another sealed envelope

that contains DS-3025? Right now, she only has a CD in a sealed envelope.

6) If (5) is true, then how does she get 2 more remaining shots in China on 09/04/2014, and get the

DS-3025 in the sealed envelop updated (since she has passed the interview already)?

7) Also, if she turns in the only sealed envelope with DS-3025 at POE,

what copy do we have remaining to submit with AOS packet?

8) Ideally, we wanted her to finish shots in China to avoid I-693 in USA,

but after passing the interview, I'm not sure if it's possible now.

I know I'm splitting hair here, but these minor details make a big difference in determining

whether she will wait couple more weeks to finish her shots and China, or just come to

USA asap and finish the remaining shots and do I-693.

Can you be more specific?

By "shot records", did you mean it was recorded in DS-3025,

and they gave you and updated DS-3025 with first and second set of shots?

I received my DS-3025 from the clinic that performed my medical exam. The clinic gave me two envelopes, my health certificate and my DS-3025.

If you are still curious probably better to post on regional sub forum for China whether beneficiaries are received DS-3025 or not.

I never heard or read an immigration officer at PoE returned DS-3025 to a beneficiary or maybe I missed out that info. Some of consulates don't give DS-3025 to applicants. That's why I advised you asked beneficiaries from China to see if this rules also apply to them.

I had my DS-3025 with all boxes were marked. And yes, I submitted my DS-3025 to my adjustment of status applications. Also I had my vaccination books with me and I sent one copy of it along with my application even though I knew it wasn't necessary.

Ask your fiancée to remember which vaccinations she already has had and determined whether she needs an I-693 from USCIS designated civil surgeon or not. I vaguely remember Nich-Nick post saying that if you have completed vaccinations and you don't have your DS-3025 and then you can write a statement that you already did whole medical with completed vaccinations and those information already forwarded to local USCIS office.

and it didn't include one that resembles that. They have recorded the shots

in what they call a "yellow book", which includes her medical exam results.

So I will double check with her on it, and also have her check with the hospital to

see if they can issue her shot records in DS-3025 form.

So I will double check with her on it, and also have her check with the hospital to

see if they can issue her shot records in DS-3025 form.

There was somebody in Philippines who emailed the medical clinic if they can have another copy of the DS-3025 because the beneficiary lost it, and fortunately, the clinic was able to send a pdf copy of it. Just a thought.

I don't think you can send shot records to USCIS in any form.

I believe it has to be in DS-3025 or I-693, and in the case you didn't have DS-3025,

the civil surgeon will either take your word for which shots you had or

they will request some proof before transcribing them to I-693.

What a messy part of the processing this is...

I only got one copy DS3025 from the medical exam, and use that to continue my shot as it mark incomplete.

I went to local health center in my country, they provide stamped in my internatinal vaccination book.. i also get Td shot in US from the New York Health center.. they printed me a paper record for the shot that day. I brought that together to the Civil Surgeon to fill out I693.

so we just need to hunt down a copy of DS-3025 from the hospital if they let us get one.

I'm trying hard to avoid doing I-693 if possible, we'll see.
