  1. DS-3025 is prepared by the panel physician at the foreign medical... it is not something that can be self prepared

    to get a vaccination history you would need to see a civil surgeon and have them prepare the I-693 Item II Part 5

    My fiancee arrived in the USA jan 15, 2010. do I still need to see a civil surgeon and have them prepare I-693 for me?

  2. My fiancee just arrived but she didn't have any of her vaccination records with her, like DS 3025. Is this a form I can fill out myself? if so where do I download this from from? I looked on the USCIS website and couldn't find it.

    I'm thinking I'll need to include copies of her vaccination records when mailing the AOS forms, right? she only has vaccination card from the clinic that's it.

    I'm a bit worried now, because I don't know how to include copies of her medicals during the AOS stage. advice please.

  3. Here's a thought of mine. Please, advice if it doesn't make sense.

    I know when you apply for marriage license you're allowed to also change your names by filling a section on the marriage application form. Is this name change effective after the marriage is done?

    I called a service center in MN, and was told I can apply for marriage license even if my fiancee has no social security number. She just need to know her passport with the k1 visa stamp in it.

    here's my thought.

    I'm thinking I'll get the marriage part of things done first so she can use her new name to apply for the social security card.

    so that she won't have to go back and change her name on the SS card after getting married.

    Does this make sense or it will cause a problem down the line?

    Yes, that is the I-94 which you will photocopy and submit. It does not have the petitioner's name because it only applies to the person entering the US.

    Well, it does have my fiancee's first and last name, plus country, on the front side of the form. then at the back they wrote my name and some 8 digit number. that's what I'm confused about. I'm not sure why they wrote my name at the back and what that 8 digit number is for?

    Anyone ever had this happen to them before?? help clarify, please.

  5. I still need a bit of clarifications on this.

    The big sealed brown envelope my fiancee received from the embassy in Ghana was taken at the Minneapolis/st. paul port. The only thing she received was a small bottom portion of the I-94 stapled to her passport. But this bottom portion is labeled "Departure Record". And the back side of the form has nothing filled out but my name ( petitioner's name) and some 8 digit number which which I don't know what that number is for.

    Is this little bottom portion of the I-94 what I need to include when applying for the AOS? Is that all the evidence I need including the marriage certificate?

  6. So my fiancee arrived and looking through her passport I noticed a few things. Please help me understand a few things:

    1. they only stapled the little "Departure Record" piece to her passport.

    2. At the bottom (same departure record form) of this piece there's questions asking for: PORT, DATE, CARRIER, FLIGHT #/ SHIP NAME

    Is this a normal process??

  7. True ONLY if your County marriage license form has field for bride's name after marriage. Some do, most do not. OP didn't disclose his location, need to ask this question at local Registrar Office, where the license will be obtained.

    I'm in Minnesota. I assume you apply for SS# before marriage license. so if you change your name at the time of applying for the marriage license, do you then go back to social security office and change your name on the card then?

  8. i went for an interview and i was given a white paper to come back with some documents .what i dont understand is some people got yellow paper what is the difference between the white paper and the yellow paper and will i be interview again ?any body with such experience or now anything about this tow papers i need to comment thanks

    I just got back from Ghana.

    My fiancee got a white paper too at her first interview asking her to bring more evidence. I guess it's better to receive a white paper than a yellow paper.

    I gathered every piece of evidence I could find, emails, chat messages, cards, phone records, and flew to ghana myself. They saw our evidence and they whine about having too much evidence ( that's the exact reaction I wanted to get from them). they asked my fiancee only one quesion, (" how often do you chat with your fiance") she answered and that was it. Visa approved. Picked it up a week later. Done deal!

    If you received a white paper, you have a second chance to prove your case. Just provide them with all the documents they're asking for. If you don't have the document, try writing a letter explaining why you don't have it. Chances are, if you have everything they're asking for, they'll ask just one or two questions or none at all and you'll get your visa.

    Important thing is to provide them with everything asked on the white paper.

  9. I have decided I'll purchase a delta airline ticket for my fiancee. she'll leave from Ghana to JFK- then catch another flight from JFK to minnesota. No direct flight from Ghana to Minnesota yet.

    my question is.

    What questions will they ask her at the airport- JFK and what are the answers to those questions? thank you!

  10. where can i get police certificate and how much will it cost me ?.

    I believe you go to the Ghana Police Headquarters in Accra for all police certificates.

    It's not expensive. probably 10 Ghana Cedis or less. Takes roughly, two weeks or so to get that but as with every process in Ghana, it can expedited if you're willing to pay a bit more under the table to the person issuing the certificate ( bribe). Sad but true.
