  1. what do we send as far as supporting evidence with the I 131 FORM, (Advace parole) and the I 765 Employment authorization?

    Do we just send two passport style photos with each form and nothing else? I have read the instructions, but am second guessing myself.

    One more question...

    So we mail the I 485, G 325A, I 864, I 131 and the I 175 forms all together in one packet to;

  2. No, do not worry, send it anyway with AOS application you will be fine, doubtful if you get a RFE over it. We sent ours in and nothing happened it was just like yours from Dr Arnold also. :thumbs:

    What I actually did after all, was simply write my A-# on top of Dr. Arnold's sealed letter (on the side which has my name and date when the form was filled of course, not the other one which has Dr. Arnold's stamp!). And as Miduvil pointed out, the A-# can be found elsewhere in the application set; almost every other form asks for it.

    Again, cheers for your help, guys! One less thing to worry about :D

On a side note here, speaking of Dr. Arnold; some of the info on him and the I-694 process do need to be updated, I think; I promise I will be updating information on this part of the process with useful tips as soon as I find some time after finishing filing; others need to benefit from our exp like we benefit from theirs! :)

    Wow, we did the same thing, no alien # or ss# put on the form. We did not do this because Dr. Arnolds office told us to absolutely fill in nothing in part 1 accept for My husbands signature.

    So hopefully we all will be fine. No RFE's. :)

  3. According to the instructions to the forms, it is 6 total, just like Mononoke said. Why would the USC need to submit any photos anyway? it's the alien's application.

    If i remember rightly you need the photos for the G whatever form. also you need less photos since you are sumbitting the application as one complete block. Having submitted the application as i described and received no RFE for more photo's i feel it is the safest bet and endorsed by the immigration lawyers. Like i said they take your photo at the biometrics and thus will have electronic copy which is much more useful for them.

    we'll follow the directions and submit 2 photos for each form = 6 total.

  4. Will my husband need 4 passport style photos for AOS? 2 for the I 131 form and 2 for the I 765 form? I also read A post where someone made a reference to submitting 6 pass port style photos for AOS. If this is true what are the other 2 for?

    I also have a question about the I 131 form in section part 3, processing information.

    Date of intended departure? Expected length of trip? What should we put here.

    Any info would be appreciated.

    Why would I need to submit photos when it is my husband who is adjusting status? :blink: Thanks for recomending the book, but I'll stick with visa journey and take the advice of the majority here. 6 pictures it is!

    umm, why would I need to submit photos when it is my husband who is adjusting status? My husband will submit 6 photos of himself like everyone has suggested.

  7. Hello everyone. It's been a while since we had anything to do with USCIS or Visajourney. Sasha and I were married in May and we've been busy working and planning the arrival of our first child. The time has finally come for us to adjust status. The guides here are very helpful, as well as the forums, but we still have many questions. I'll try to keep it as simple as possible. Thank you for any information you can provide.

    1) I understand we do not need to provide an I-693 Medical Examination of Aliens Seeking Adjustment of Status, since she came here with a K-1 visa and had a medical exam within the last year. However, USCIS requires a vaccination supplement (I-693A). We cannot locate this form anywhere on the USCIS website. Only the full medical form. What to do?? **UPDATE** I've located the supplemental part "A" for the I-693. Can I print this out and take to the civil surgeon?

    2) All civil surgeons that we've contacted say they will only do the vaccinations with a full medical exam and 200 bucks. Despite what we've read here on VJ from members who insist this is not neccesary :wacko: . What to do??

    3) Sasha is pregnant. And she should not receive some of these vaccinations while in such condition. Can this be waivered? What should we do? We can't wait until after the delivery to file this adjustment. It needs to be done now. Both USCIS officers we spoke with over the phone seemed pretty clueless to this question. Should we simply submit a doctor's letter stating she is pregnant and cannot receive the vaccinations? My gut tells me this will just trigger an RFE. What to do?

    4) It seems some members have sent the work and travel authorization documents to different offices than their actual I-485. Are we supposed to send everything in one package to Chicago?

    5) Part of the checklist for AOS states that the I-94 needs to be valid. However these expire after 90 days of arrival. We opted to wait for filing since it was not required to file for AOS within the first 90 days. Only marriage was required in that 90 day period. Sasha's expired 2 weeks ago. We have seen many couples who chose to wait long periods of time between marrying and filing. Will this cause a problem? she is here legally because we are married and she is allowed to be here through the process. And, of course, she did enter legally through JFK.

    Well, that's all I have for now. But we want to thank everyone for their help. Its so damn confusing and the cavemen that work at USCIS are quite robotic in nature, not to mention VAGUE. :P

    We are ready to apply for AOS as well. The information that I have gathered regarding the vaccination supplement has been vauge. From what I have gathered, the form I 693A is no longer being used. The appropriate document is the I693. You can print this document off on the US Immigration website. We contacted Dr. LE Arnold in Texas who is a civil surgeon to complete my husbands vaccination supplement. The cost is $20.00 and the process is pain less. You do not have to deal with civil surgeons who are going to charge you for the full cost of another medical. And yes, you are correct. Sasha does not need a full medical as she came in on a K1 and has not been over a year yet. I would suggest contacting Dr. Arnolds office to transcribe her vaccinations. Soooo easy, as it should be. On a side note; I have also read that the cdc is now requireing tb test etc. for adjusting status. When we contacted Dr. Arnolds office, we inquired about this. We were told that they have not heard anything about this new requirement. So wether or not the tb test is actually required remains a mystery. If I am understanding correctly, the tb test is not required if a full medical was performed after May 1st of this year? I could be wrong about this.

    As far as Sasha being pregnant, if you choose to use Dr. Arnold I would get something from her OBGYN stating that she is pregnant, and send this proof to the doc in texas. This info could possibly be put on the vaccination supplement to support the fact that she can not have certain vaccinations.

  8. Does the I-864 form have to be notorized? NO Do we need supporting evidence such as the letter from our employer, tax transcripts etc.? YES that would help a lot

  9. so I shouldn't bother working until after I have sent away my complete AOS package including the I-765? Is it just easier to wait?

    and what do I need to open a bank account in the meantime?

    Do You have a temp EAD STAMP? EAD card? If you do, then the choice of wether to work or not is all yours. Personally, if I didn't have to work I woldn't. :) If you don't have a temp EAD stamp or EAD card, then you can't work.

    Yesterday I was getting together all the paper work to send off for my husband to their office and realized that we may not have his DS 3025 form. All we were given was a form with all of the vaccinations that he received. At the top of the sheet it says... VACCINATION DOCUMEMTATION MEDICAL WORKSHEET TWO

    Then it has his name, exam date, DOB, passport number, alien case number...Then the list of vaccinaations received. Then at the bottom the doctors signature.

    Nowhere on the form does it say DS 3025.

    Is this the DS 3025 worksheet?

    He had his medical done in Canada.

  12. neither have I undergone a tb skin test on my medical, it was just an x-ray. Blood test are required for HIV and drug test which should be done during the medical for visa. Are there any note attached/written on his vaccination form that he needs to comply after arriving here in US? If none, then he just need the supplementary copy of his vaccination.

    No there is no note attached to the visa or anything saying that he needs to get the tb test and blood work. I have went to the USICS web site and I have also read post on here saying that things have changed with the I693 form. The centers for disease control are saying that a person now needs to have a tb test and blood work completed as well as all the rest of the vaccines etc. (which my husband allready has).

    noone can seem to answer my question or shed light on this matter. I keep getting conflicting answers. We do not know if my husband shoulg go and get the tb test etc. (going by the new info. on the I 693 directions, or just take the vaccines that he currently has and have them put on the vaccination supplement form).

    Do we or don't we? Do we submit the I 693 WIthout the tb test and blood work? Are we prone to get an rfe? Please please somene advise us....

    We need clarification.

  13. I recently came across this post..... I am wondering if anyone can shed some light on it.

    Another reason for a medical RFE.... you are required to have a TB skin test, not just the X-RAY test done in other countries. We got an RFE because of this, so make sure to send the results of a skin TB test if you have not done so already.

    My husband never had a tb test or blood test at his medical examination. We are ready to adjust staus, and are going to have Dr. Arnold in texas to complete the vaccination supplement.

    Should he go and have a tb test and blood work done? Or will the vaccinations that he received at his medical be enough?

    Ok...I'm confused. I thought you had to have the EAD to work. Are you saying all you need is a ssn # and the I94 to work legally?

    I've really missed something.

    Nope, you need an EAD to work. I don't know where T.O_2_FL got their info from but it's wrong.

Am I undederstanding the new form I-693 correctly....my husband will now have to have a TB skin test and blood work? all of this will have to be added to the the I-693 worksheet as well as the vaccines he currently has received?

    So it is pointless to go with Dr. Arnolds office in Texas to complete the vaccination supplement to adjust status because he now needs more shots?

  19. There is no 693A anymore, USCIS as updated the I-693 with directions for K-Visas, it has a vaccinations section and directions to do just that section for K-Visas.

    The special instructions are for K visa applicants who are submitting the I-485 after one year of admission to the USA....

    NOTE: If you are applying for adjustment of status at least one year after your first admission to the United States as a refugee or as a "K" or "V" nonimmigrant visa holder, see Questions 1-3 in Section III, Frequently Asked Questions, before proceeding any further.

    My husband has not even been here for 3 weeks yet. so the special instructions do not apply to us. All we need to know is where to get the I 693a, or I 693 form, or whatever it is that we need.

    Can someone make this clear?
