The I-485 has a section for "Other Names You Have Used Since Birth". Your wife listed her maiden name in that section, right?

In case the flight is canceled after checking in, have her go back to the CBP pre-clearance office before exiting airside and ask the CBP officers to assist with clearing her entry record.

The final CSPA calculation will be done by the consulate. No special form required from the petitioner or beneficiary. All the necessary information is already on the petition, petition approval notice, and visa bulletin.

Just wait for the CEAC status to show "Issued". Whether it currently shows "Ready" or "Refused" does not really indicate if the visa will be issued soon.

The I-94 record will be generated upon CBP inspection on US entry. No need to make a special request for it. Just check the record online maybe a day after arrival in the US.
