Will USCIS still be advised she re-entered because all her USCIS documents for AOS are instead her married name? I'm unsure if we have to send some notice to USCIS or if that's all taken care of because it will be matched up with the passport information.

The I-485 has a section for "Other Names You Have Used Since Birth".  Your wife listed her maiden name in that section, right?

Our concern is if she gets through US Preclearance at the Dublin POE and has thus "used" her K1 Visa, and then the flight is canceled due to poor weather, what do we do?

In case the flight is canceled after checking in, have her go back to the CBP pre-clearance office before exiting airside and ask the CBP officers to assist with clearing her entry record.

    What steps to do?

The letter states what was wrong with the I-485A submission.  I-485A needs to be submitted with or after an I-485.  This is also clearly stated on page 1 of the I-485A form instructions -- https://www.uscis.gov/sites/default/files/document/forms/i-485supainstr.pdf

You cannot adjust status based on filing Supplement A alone. You must also file Form I-485 and be eligible for a visa under a family-based, employment-based, special immigrant, or Diversity Visa immigrant category. Supplement A and Form I-485 are used to determine whether you qualify for INA section 245(i) adjustment.

Who May File to Adjust Status Under INA Section 245(i) Using Supplement A?

Use Supplement A if you are applying to adjust status under INA section 245(i). You may file Supplement A when:
    1. You are filing your Form I-485 at the same time; or
    2. You previously filed your Form I-485 and it remains pending.

who calculates CSPA age and whether it is applicable for the derived beneficiary or not? NVC or local consulate? or does the petitioner/primary beneficiary need to prove/fill out a forum?

The final CSPA calculation will be done by the consulate.  No special form required from the petitioner or beneficiary.  All the necessary information is already on the petition, petition approval notice, and visa bulletin.

Does this mean that my visa will be issued soon?

Just wait for the CEAC status to show "Issued".  Whether it currently shows "Ready" or "Refused" does not really indicate if the visa will be issued soon.
