If you don't meet, then I had used this form when i first arrived in US , and didn't fill out any foreign tax number. I also updated the information with bank once i was resident for tax purposes, and told them to eliminate this paper since it wasn't applicable anymore.
Thank you! I have submitted the form again. Still waiting if they approve it or not. Told them about my situation and still required the W 8-BEN form.

I went back and forth with the W-8BEN with USAA for two months last year. The final version that was accepted had this:
- Permanent residence address (box 3) = address in Ecuador
- Mailing address (box 4) = PO box in Ecuador (but if the permanent residence address also doubles as a mailing address, this wouldn't be needed)
- U.S. taxpayer identification number (box 5) = blank
- Foreign tax identifying number (box 6) = appropriate number from Ecuador
- Reference number (box 7) = wife's recently-assigned USAA number
- Part II = blank (due to no tax treaty between the US and Ecuador; this may be different for you)
Note that by the time I sent this final version, my wife was no longer in the US. My understanding is that they did want the foreign address in box 3 even when she was in the US adjusting status.
Things that caused problems with the various versions that I sent included:
- Putting a US-based address in box 3
- Not putting the foreign tax identifying number
- Listing a US-based mailing address with the foreign permanent residence address
- Using my wife's married name instead of the maiden name on her passport
A few times they sent back a form called the "Non-resident Alien Additional Information Worksheet" asking me to explain why I answered certain questions the way I did. For the last version that they accepted, I had to explain why the mailing address wasn't the same as the permanent residence address.
No one on the phone could give clear guidance, so instead I would submit the form (online) and then wait for a response by mail to see if it was accepted or not. If felt like a bizarre game of "Go Fish" at times. Frustratingly, even after I corrected some of the issues they pointed out, later correspondence would continue to reference the now-corrected issues as if they were still looking at the first W8-BEN that I sent.

Hello, has anyone here filled out a W-8BEN form? I just need help on filling out the form. They returned it to me due to some errors. They are requiring my foreign tax number but the customer service rep from USAA that I talked to told to leave it blank. Kind of confused. Thanks in advance!

Hello everyone! Help please! I haven't applied for my Social Security Number but we are already getting married tomorrow. Is it okay to get married first without the SSN? Can I apply after I get my marriage certificate and can I apply for AOS without my SSN? Will there be a problem if I get married first, apply for SSN then apply for AOS?

I am assuming they got the medical on the 15th when the case was updated, but my case still is refused and it has been quite a while now. Seems like people who had interviews/medicals around my time got admin processing/issued status much quicker
Hello! We have the same case. I had my Medical on the 8th at O'Connell Street Clinic and they said they sent the report on the 9th but when I had my interview on the 14th, the officer said they haven't received it yet. The officer also said that I should have my visa within 1-2 weeks but it's been a week now and my case status is still refused. I emailed them this afternoon awaiting for their reply tomorrow.

I just received an email back from Sydney. They said they have all the necessary docs and it’s with the officer but it takes usually around 15 days to process once received. So I guess worst case it will be done by the 27th. Our interview was the 6th. I’ll keep you posted!
Wow! 15 days! The officer who interviewed me told me that I just have to wait for 1-2 weeks for my visa. I'll e-mail them on Thursday if I still don't get any updates. Thanks so much!!!!!

I did end up sending them an email today because I’ve already booked flights and starting to feel a little stressed. I’ll let you know if they respond or anything changes! Keep me posted as well
Awww! Thanks for sharing your situation. When did you have your interview? I will really appreciate if you keep me posted so I know if I need to email them as well. I really hope we get our visa soon, especially for you as you have booked your flight already.

Hello everyone! I had my Medical at O'Connel Street on the 8th of October and had my interview on the 14th of October. During my interview, the Consulate still hasn't received my medical report. The guy who interviewed me reassured me that I have nothing to worry as they will approve my case once they receive my medical report.
How long does it usually take? it's been like 12 days since my medical. The clinic said they sent my report on the 9th of October but still my visa case status is Refused. Should I make a follow up or just wait?

They emailed this afternoon, at 430 pm Sydney time, saying the interview is TOMORROW, which of course my husband won't be at because he's in the middle of Queensland... (Not to mention no medical since they hadn't given us ANY date prior). I just can't with this process...
Sorry about that. But what additional documents did they request?

The new form that you create will also have a confirmation page with the bar code and that is the one you bring to the interview. If it were just a simple address change then you could update at the interview but you have other items and I doubt the CO would like to spend 10 minutes updating your DS-160, Online Nonimmigrant Visa Application. So do a new one.
Hello, I tried to retrieve a DS-160 application before when I applied for a Tourist Visa and I just edited it and I was provided a new code. Can I do that again and retrieve an application so I can just edit the address and my employment details? Thank you! @Greenbaum

Hello! I just wanted to confirm what hair color should I put on my Biometrics appointment form if I have recently dyed my hair? I assume I should put the natural hair color but others have been saying to put the existing hair color. Thank you very much in advance.
