You should call NVC!! Although they should have included that information on the email I believe. Today my fiancé actually called NVC and we got our Case & Invoice #!

What visa are you applying for exactly... If I were you I would find the actual application online and compare it to whatever sample you are using. You shouldn't leave anything blank in any section, I put NA in any box that didn't relate to us. If you are not married, they are not your spouse.
