He can get an idea the reason of denial if he know the exact question that the officer ask and what answer given by his fiancee. Basically for this case, the officer doesn't believe their relationship is real. So, the only way to overcome it is by having more face time and provide more relationship evidence next time.

DS-160 doesn't need NVC case number. It just need petition receipt number.

Do you think person who are in the middle doing "removing the condition" the only who can be inside US? Sorry, but I don't understand what you are trying to say. If you referring to my timeline, you will know that it is up to date. If you noticed that the OP question was answered, why need to focus on what I replied? Cannon fodder? Maybe it is referring  to yourself. He answered my replied and I answer back. That's all. Not sure why you jump into the  conversation and now said things about "Cannon fodder". I'm not sure why the terms used is too important. In US use "letter size paper" but, some of the country call it "A4 paper". So, what is wrong with that? 
 
My mistake, I didn't notice that he is removing a condition. I share my experience what I did on my K-1 Visa. That's it. 
 
 

It is not necessary, but it will help if you are lack of relationship evidence. My fiance and I meet just once before we filed our petition (I know 1 visit is too short). Instead of doing 2nd visit while waiting for our NOA2, I suggested to him that I want him to be there during the interview since we just have one visit. He agreed with that, and I believe that by having him there with me, it really helps me a lot. Furthermore, we have more relationship evidence since he came. If you believe that you have lots of relationship evidence (especially time spend in person), it is okay for you to not be there. 

My family reject my relationship with my fiance. I have a guilt when I need to choose between my family and my fiance. I closed to my family before and it is such a difficult decision to made. I need to move out once i made that decision. When I first move out, non of my family member contacted me. I'm all alone. The day I fly to States, none of my family member send me to the airport. My youngest sister did texted me on that day. She is the only one who still keep a contact with me. Is it easy? Nope... Did I feel guilty by choosing my fiance and leaving my family? Yes. They might misplaced your birth certificate. I'm not sure whether the date is the problem.

She still need police certificate from Kuwait, but she doesn't need to apply the new one.That is why people applying their police certificate before they leave the country. Police certificate is needed for each country that the beneficiary reside for 6 months or more.

She already has one but it is expired. So, I suggest for her to still bring the expired police certificate from Kuwait just in case they request for it. 

If you are referring to submit the petition, I-134 is not required during that time. It needs to be submitted during the interview. You should submit 2 I-134 during the interview (one is fill up by you, another one by co-sponsor)

It is the best for you to do it now, before the interview. 

That is what happened to us too, we called each other "hubby and wifey - even though we are not yet married", that is why we really checked every single evidence before we submit it. It is too risky and we are not willing to take that risk

You can try to contact (email or call) US Embassy at your country. I do that when they reschedule my appointment from 6th to 20th. The worst part because I already bought a ticket to States (18th-shouldnt buy unless you have visa on hand, but we take a risk anyway because we want to fly back together). So, I contacted the embassy at my country, they push the date to 8th. It is not hurt to try.

AP is needed even when it is an emergency situation. 

You need to be more specific on what visa you are apply for (since each visa have different process and different processing times.) and the beneficiary country
