Yeah, Ukraine, similar to Russia do not accept the marriage certificate as a legal name change document.  You might try looking into one of those online legal services to see if you can effect a court ordered name change to satisfy the Ukraine immigration folks.

In the US your marriage certificate is all you need. If you're trying to change things abroad you will have to follow the laws there. THey may not allow  name changes just based off marriage and you may have to use your maiden name or get it legally changed there.

Thank you for your encouragement.  This is our first RFE; I've been organized, and it's frustrating for that not to pay off.  I'll do my best to take your advice

I'm sorry to hear that.  We got an RFE once during our removal of conditions.  It sucks.  I think you re-sent everything you could and more.  All you can do is just wait now.  Just know: none of this process will be very logical or efficient.  Just roll with it, be patient and try to distract yourself with other things because it can be very consuming.

Yes. You can download the deposit slip online and she can go to the nearest BPI branch. The visa fee will be pain in peso.
If she pays after 3pm, she can schedule her appointment at 1PM the day after.
For the medical, it says on their website that they suggest doing it 7-10 days before the interview (I am trying to find out if it is okay to do this a month before but I am not sure yet). You will need to bring your appointment letter btw
