It says, you may submit one check or money order for both application and biometrics service fees. No additional fee for I-765 and I-131 if processing with I-485. Make the check or money order to U.S. Department of Homeland Security.
http://www.uscis.gov/files/form/i-485instr.pdf
Good luck!!
Thank you so Much for your help. God Bless.

Hi, I'm just going to ask if I still need to provide separate money orders for biometrics of the two forms or only one will do?
Example, biometrics for I-485 is separate from the biometrics of I-765?
Thank you so much..

I think you mean the DS-3025, and no, it is not the same form as the I-693. You are given the DS-3025 at the time of your K-1 medical. If you are missing vaccinations after the medical, then you will need to see a civil surgeon in the US and get the missing vaccinations and have the civil surgeon transcribe all your vaccinations onto an I-693 to send in with your AOS paperwork.
You need at least one in a series of MMR, Tdap/TD, varicella(or VH marked on the DS-3025 to indicate you had chickenpox already) and the influenza(flu shot, which is required during flu season October - March).
If you had all the required vaccinations and they were marked off on the DS-3025, you can just send a copy of the DS-3025 and skip the civil surgeon. Check this post for details about how to know if the DS-3025 is good to go without seeing a civil surgeon: http://www.visajourney.com/forums/topic/335754-ds-3025-complete-or-not/page__p__4971318#entry4971318
Hi, thank you for your reply. this is a big help for us.. God bless..

Hi Guys,I just want to ask, what is the I-693 form? is it the same with the DS-2053?
Do I need to complete all the required vaccinations before filing because I was given 3 shots of different vaccines but still, additional vaccinations are needed?
thank you and Godbless..

I answered you here: http://www.visajourney.com/forums/topic/396412-questions-before-filing-aos/

Hi, We just got married and I'm now working for my adjustment of status. I just want to ask these questions:
1. Where can I get my Alien number? Is it something to get when I already have my temporary green card?
2. What surname should I type about the "Information About Me", is it the surname of my husband or still my maiden surname?
3 Where will I send the completed forms/package?
Thank You so much and God Bless.

Thank you everyone for your response. I was wondering why the pictures that my fiance submitted for I-29f was out from the sealed envelope. Is that a problem?
-
Most people only submit one copy of the I-134 and supporting documents. The consulates tend to follow the guidelines for the I-864 even when dealing with the I-134 so the instructions for the I-134 are not really followed all that closely. Your consulate probably did not require more than one copy of each document. If the consulate took your forms and approved your visa, then all you needed was what you gave them. You have your visa and sealed envelope. You are good to go.
Thank you so Much:)

CONGRATULATIONS !!!
Consider posting your review of the consulate interview in your timeline so that others may benefit from your experience.
Wishing you continued good fortune on your immigration journey.
Thank you. but how will I post it?

Definitely thinking too much. All the stuff they need to see is INSIDE that envelope.. your entire K1 package from beginning to end.
You don't need copies of the I-134
You're right. Thank you so much. I became paranoid when I read the instruction regarding submitting duplicate copies of all the evidences that support the I-134 which i failed to do during the interview because I only submitted one. hehe. they didn't give me 221g form for that.

You already got your visa so it should be no problem for you. Unless you bring in illegal drugs
ConGraTuLaTions
Hehe. thank you. of course not. I'm too afraid to do that..

Huh? If you have the sealed envelope, that's the only thing you should give at POE. Do not tamper with that envelope and hand it over. All the information the CBP officer needs, is in there.
Yes, I have it already and I don't have a plan of opening it because I know it's forbidden but my concern is i just provided 1 copy of supporting documents for I-134. Do you think it will affect my entry?
You got the visa - rejoice ConGraTuLaTions .
You should be fine at the POE
Wow, thank you. maybe I'm just thinking too much. hehe.

Hi VJ's. My Visa was already approved and I already have it on hand, however, I just noticed today that I made a mistake on filing I-134 Affidavit of Support during my interview. I missed to photocopy the supporting documents of that form that's why, I only submitted 1 copy of each.
Do you think I will be denied entry because of that and what are the other possible reasons for denying to enter US?
Thank you and God bless.

Hi everyone, I need help and advice from anybody on this forum please. My fiancée has filed for the K1 fiancé visa and l just realised am hepatitis B positive. I just want someone to help me with a valid and authentic information if it is going to affect my visa or best saying, will l be rejected or refused a visa. Is hepatitis B amongst the categories of diseases of public health significance. Thanks for your enormous contribution.
You or your fiance can write the US Consular officer or the examining physician that your USC fiance is aware of it.
If you're already vaccinated, it should no longer be a problem.
check this link out: where someone already asked this question
Check out this link http://www.visajourney.com/forums/topic/222186-hepatitis-b-reason-for-visa-denial/page__view__findpost__p__3385132
Once a person was infected by Hepa B virus, vaccine will no longer effective because it is only to protect a person from being infected and not to treat it.

Hi! thank you for the info... BTW when is your interview date?
I'm very happy to help.
My interview date was on August 14, 2012.
-
During the vaccination interview, the physician will decide on what vaccines should be given to the applicant. 3-4 shot is usually given here but those shots do not complete the list. Once the medical exam is completed, the applicant will be given list of vaccinations that were given and should be given. you can base the other needed vaccines when required by the USCIS and you can have it in the US. If some vaccines were given to you before, bring the record to St. Luke's so that they can include them to the list of vaccines given. Good luck and God bless.

I had my Medical on August 1, 2012 and my interview was on August 14. Everything went fine during the interview. The minimum schedule for medical exam is atleast a week prior to interview not too early. 2 weeks is safe. good luck and God bless

Yes, once you have a case number, you can set an appointment. Pay the Visa fee at BPI bank then call the US embassy or log to http://www.ustraveldocs.com/ph/. No need to wait for any packets. Just download it to this link: http://manila.usembassy.gov/wwwh3224.html and this will tell you all the required documents and requirements that you should have in the interview.
Good luck and God bless.

No need to make an itinerary but sometimes, the Consular Officer will ask you about when you are you going to US. You can say that you will finalize it once you get your visa. God bless and Good luck.

Hi. I just had my medical last August 1. It was raining that time that's why the standard 2 days of medical exam became 1 day only because applicants are few that time.
On day 1, you will know if you pass when they schedule you to have your vaccine on the 2nd day.
If you know the LRT Line 1 United Nations Station, you can walk from there and ask the people where Bacobo St or St. Luke's Extension Clinic is located and they will help you to find it. You can also ride on a Pedicab, maybe it will cost P20.
Good luck and God bless to your Medical Examination and Interview.
