  No worries O_g ("OG" I hear ya girl!!!)...we'll be there for you! @Debbie, quit worrying about that form. I sent mine out electronically on the 10th or 11th. I got a checklist for it (yep, a real one) on the 20th and I sent it in again from Divine's email. The next day or two I got billed for the IV.
