Does anyone know where I can find a civil surgeon? I live in the KC area. Also, when we set up the appointment, is there a form we have to bring or will the civil surgeon have it? The consulate-approved doctor just gave my wife a list of the vaccinations she got before the interview in Lima. I've got everything else ready for AOS, but I'm pretty confused by the whole I-693 thing. Sorry if this is a repeat question...I tried to search through previous posts but couldn't find the exact answer I was looking for.

Also if adjusting status from a K-Visa you dont need the full medical if the medical for visa was less than a year ago, you only need supplement, DO NOT let the Civil Surgeon soak you for a full medical.

The above link provides a copy of the supplement to take to the doctor as well as a link to the CS locater.

Thank you for the responses. It's starting to become clear. One more question though if you don't mind...my wife had her medical exam about 2 months ago, so we're within the one-year limit. However, she didn't get anything from the embassy or the doctor in her country regarding her medical exam...only the record of the vaccinations she received. She called the doctor's office that did the exam in Peru and the doctor said she can't forward information to us. Will USCIS have record of the exam she had done that the consulate in Lima has or will we have to do a full medical exam again?

the results form the medical exam in lima were in the brown envelope your wife had to give the officer at her POE. the only thing you need is the vaccination sheet she got from the doctor in lima and get that transfered to the I693a.

heres the adress of a doctor that will transfere that info for just $10 every other registered civil surgeon will make you pay a ridiculous fee!

Thanks a bunch for the information! Not only has visajourney saved us a bunch of money in lawyer fees but I'm guessing you just saved me at least $100 or more in doctor's fees :thumbs:

Lol...Well...the medical examination at a civil surgeon's office is around $500
