Statement from employer for I-134

I was just wondering if there is a template or example for an employer letter to submit with I-134.

What exactly did you have your employer include in the letter?

If you could share your experience, it would be really appreciated.

12/27/2007---USCIS lost a part of our packet and returned the rest of the packet with denial letters

12/31/2007---AOS, I-765, I-131 received by USCIS

01/04/2008---NOA issued by USCIS

01/08/2008---Biometrics Appointment letter issued by USCIS

01/26/2008---Biometrics Appointment Date

01/28/2008---Touched

01/23/2008---Initial Interview Request issued

02/15/2008---I-485 touched (hope no RFE is coming...)

02/23/2008---I-131 Approved

02/28/2008---I-765 Card production ordered

03/03/2008---EAD card received in mail

03/04/2008---I-131 received in mail

03/11/2008---Initial Interview Appointment---APPROVED!!!!!

03/17/2008---Welcome notice received in mail

03/21/2008---Green Card received in mail!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

Timeline

It can look something like this....(I found this here at this site):

"To whom it may concern: (first name,last name) has been employed

full-time with the XYZ Company since 200_ as a (name of your job), and is an employee in good standing with an

annual salary of $XXXX. Signed, John Doe, Supervisor (or whatever title)"

The letter should be on company

letterhead.

11/2004 - Met in Brazil

09/2006 - Apply for K1

03/2007 - K1 approved

04/2007 - Apply for AOS & EAD

07/2007 - EAD approved

01/2008 - Conditional Residency approved

11/2009 - Apply to remove conditions

02/2010 - Permanent Residency approved

11/2010 - Apply for Citizenship

03/2011 - Citizenship approved

07/2011 - Moved back to Brazil

Timeline

It should say the date when it was issued,How long You have been working for and how much money you made in last year,or monthly wage.

will be also great if it says something like-he is a stable employer and very valuable for the company.

Will be better if it is signed by boss and stamped or notarized.

The letter should also state if you are a temporary or permanent employee.

My letter was short, sweet, and to the point...my boss just signed it - I wrote it and e-mailed it to her.

We used the same letter for AOS.

11/2004 - Met in Brazil

09/2006 - Apply for K1

03/2007 - K1 approved

04/2007 - Apply for AOS & EAD

07/2007 - EAD approved

01/2008 - Conditional Residency approved

11/2009 - Apply to remove conditions

02/2010 - Permanent Residency approved

11/2010 - Apply for Citizenship

03/2011 - Citizenship approved

07/2011 - Moved back to Brazil

My employer put my name, my position, how long I had been with them and my annual income on the agency's letterhead.
