I-693

I’m confused about the requirements for a medical exam and the language of the I-693 form. I’d appreciate any help I could get. 
 

My grandson’s new wife came over with a K-1 visa and, now that they are married, is filing for adjustment of status. She had her medical exam at St. Luke’s in Manila and is well within a year of that exam. My understanding is that she does not need to undergo a new medical exam. Is an I-693 required at all?  The instructions seem to say it is required only if the vaccination record was not properly completed but does that mean none is required if we have no reason to believe that the vaccination record was not completed properly?

Check DS-3025. If box "K Visa applicant voluntarily completed vaccination requirements" is checked, then no I-693 is needed
