Hey Guys, 

 

you probably get this question a lot because it seems unclear to me. My wife entered the U.S. on a K1 and K2 for her son. About to file AOS but it’s saying I don’t need to file I-693 medical form but instead I could send in a DS-3025 vaccination worksheet, however my wife says she and our son never got that when they did the medical in Jamaica. 
 

should I just send in the application without medical or vaccination worksheet? 
 

please advise the best way forward.

The DS-3025 copy you would send in is really just a reminder to the adjudicating officer that she had her medical done overseas and to look for it in her A-File. Since she doesn't have her copy, you can just mention in the cover letter table of contents something like this:

 

I-693: No I-693 included because as I am adjusting from a K-1 visa, I completed my immigrant medical exam overseas on <Date> and medical report was handed to CBP upon entry to the US at <PoE>  on <Date>

Hi! I had the same issue! I was confused, because I never received any forms of anything after my medical. 

 

I ended up sending them an email requesting this form, with all fhe information I had send them when making the appointment for said exam. 

They were so kind to email me this form, which I then printed here in the states to include in the package for the AOS

My medical was in the netherlands, but I got their response including the exam results within 2 days. 

 

so, just contact the office where the medical exam was done and request the document. I hope this helps! ✌🏻😊

Did your wife recieve a sealed packet that she handed over to CBP upon entry? That contains the original DS3025. I'm assuming so, as it's the norm, but sometimes people miss this part.

 

If yes to the above, she should try and get a copy of it from the medical examiner she saw to include in her AOS. But if she can't, it should be OK as USCIS should have recieved the original in the packet handed over to CBP. 

 

If she did not recieve a sealed packet, then you may have to redo the medical.

Thank you guys for your responses. Yes she did get a sealed package which was handed over the CBP when she entered the U.S. We have also since emailed the medical facility where the exam was done, if they respond with the DS-3025 I will send it, if not I will just add it to the cover letter as @Edward and Jaycel suggested above.
