Hey Guys, 

 

you probably get this question a lot because it seems unclear to me. My wife entered the U.S. on a K1 and K2 for her son. About to file AOS but it’s saying I don’t need to file I-693 medical form but instead I could send in a DS-3025 vaccination worksheet, however my wife says she and our son never got that when they did the medical in Jamaica. 
 

should I just send in the application without medical or vaccination worksheet? 
 

please advise the best way forward.

The DS-3025 copy you would send in is really just a reminder to the adjudicating officer that she had her medical done overseas and to look for it in her A-File. Since she doesn't have her copy, you can just mention in the cover letter table of contents something like this:

 

I-693: No I-693 included because as I am adjusting from a K-1 visa, I completed my immigrant medical exam overseas on <Date> and medical report was handed to CBP upon entry to the US at <PoE>  on <Date>
