Where can I get this form (DS-156K) Is this specific to the embassy in which you are applying?

I am petitioning through the Nigerian embassy

This specific form is not customized as far as I am aware. That being said, it is always a good practice to check with your specific embassy first to find out what they use/prefer.

Here is one version of it. I switched browsers and probably have a link to the government version of the DS-156K as well... Are you certain you need this form in addition to the DS-156?

Different posts expect the DS-156K to be filled out slightly differently. In Russia, for instance, applicants are to fill out the DS-156K in Russian, but the DS-156 in English.

I don't know if Nigeria has any quirks in this regards or not.

Well since Nigeria's national language is English - all forms should be completed in English.
