I-130 Form Questions

I'm a USA Citizen, sponsoring my wife. I have these following questions, appreciate help:

1. Do I need to include my wife's birth certificate with the I-130 and I-130A submission? 
2. How do we show employment changes that resulted due to merger, my wife worked for X company until June 2021 and then the company was bought by company Y
3. Part 4, Question 57. My wife is a Permanent Resident of Canada, but she is a Indian citizen. It states to write her foreign address, is this her Canadian address or her past indian address?

Up to you, but USCIS doesn't really care about the beneficiary's employer mergers/re-organization.  I suggest simply putting 1 entry for <X company> for the entire duration of your wife's employment there.

It refers to current physical address outside the US.  If the current address is a Canadian address, enter "N/A" or leave item# 58a-f blank.
