DS-260 Question

I am petitioning my mother. And she is already approved for I-130 and her case is in National Visa Center (NVC) I am now completing ds-260.

 

There is a question in the 'Previous US Travel Information' section asking if she had ever been refused a US visa.

2 years ago My mother applied  for a tourist visa twice and both time she got denied  we still don`t know the exact reason why she was denied both times but at that time  I wasn't working and haven`t become a citizen yet and the officer told her to reapply again.

My question is, should I mention that she got denied 2 times or It is ok if I just just mention  that she was denied the visa without mentioning how many times she was denied .

 Because the question seems like a general question.

 

My other question is what should I write for the reason why she was denied , we don`t know the reason why she was denied  both times but we noticed she made a mistakes on her  DS-160  form at that time but still don`t know if that was the reason or no .

You always need to answer the truth because they know exactly how many times.

The why is probably lack off strong ties.

What exactly they told her?

Just answer Yes to "Have you ever been refused a US visa [...]?"  No need to say how many times.

When she was denied, the consul officer probably gave her a paper with a reference to the section of US immigration law that is the basis for the denial.  If she lost that paper or really cannot remember what was written on it, just answer Reason Unknown or something like that.
