Hi! We're completing the DS260 form and have 2 questions:

 

1. Should we answer yes or no to the two following questions:  "Have any of your US visas ever been cancelled or revoked?" and "Have you ever been refused a US visa, been refused admission to the US, or withdrawn your application for admission at the port of entry?". 

My husband was found ineligible the first time he tried to apply for an F1 Visa but then approved and issued the second time he tried. (He was ineligible the first time due to not demonstrating strong ties in the US.) After completing his education in the US, he applied for an H1B Visa which was granted. He worked in the US for almost 10 years until his H1B Visa extension was denied due to his position not qualifying as a specialty occupation.

2. Where can we find my husband's Alien and H1B Visa numbers? 

We found an Employment Authorization Card with what seems to be his Alien number starting with an A and followed by 9 digits. Is that it?

In regards to his previous H1B Visa, his employer and lawyers always submitted the papers and only gave him receipts. Where can we find his H1B Visa number? Is it the same as the F1 Visa number he had before converting it to an H1B?

 

Please help! We only need to accurately answer these 2 questions to be able to submit the DS260 and other documents. Thank you! :)

1. the question is pretty clear, you either tell the truth or you don't. I recommend the truth since it's likely they can find out.
