Could anyone please explain how to file pat 11 answering a question N22 about addresses I've resided at since becoming a perm.resident?

Do I start from the most recent address? how do I fill out the dates?

Example:

 

HOME NUMBER 2, Florida

Present - 07/06/2018 

 

HOME NUMBER 1, California

01/01/2017 - 07/06/2018

 

Would this format be correct? for example?

Correct, start with most recent one.

07/06/2018 - Present would be the correct way for your most recent residence.

other format is good.

2015-06-16: I-130 Sent

2015-08-17: I-130 approved

2015-09-23: NVC received file

2015-10-05: NVC assigned Case number, Invoice ID & Beneficiary ID

2016-06-30: DS-261 completed, AOS Fee Paid, WL received

2016-07-05: Received IV invoice, IV Fee Paid

2016-07-06: DS-260 Submitted

2016-07-07: AOS and IV Package mailed

2016-07-08: NVC Scan

2016-08-08: Case Complete

2017-06-30: Interview, approved

2017-07-04: Visa in hand

2017-08-01: Entry to US

Also, do I need to enclose a letter ?

I am filing a joint Form I-751, Petition to Remove Conditions on Residence, with my spouse,

We have enclosed Form I-751, a check for $[USCIS fee amount], and copies of the permanent resident cards ...

. Please also find the following documents enclosed to verify the status of our marriage and ongoing relationship:

Copy of our marriage certificate
Copy of our apartment rental agreement with joint occupancy
Copy of the birth certificate of our child born out of this marriage
Copies of the tax statements for the previous two years (2015-2016)
Copies of statements from our joint checking and savings accounts for October 2015 to present.
Photographs from a family vacation to the Grand Canyon in June 2015, my parent’s anniversary in January 2016, my wife’s birthday in May 2016, and a vacation to Las Vegas in November 2016
