DS-3025 is the medical exam form an authorized doctor will complete after examining you. Bring your records of vaccinations you received in the past or bring documentation showing blood titers (if documentation unavailable). Worst case, get all the necessary shots at the medical exam - should be included in the price.

There is a list on cdc but the panel doctor (approved by US) will give you what you need at the immigrant visa medical anyway. There are a number of vaccinations that adults are age exempt, some people try to save money by going to get a bunch first and end up unnecessarily getting ones they don’t need.

 

When you are scheduled for interview depends on your case number. Britsimonsays site has a lot of info about the process, how to judge your number etc.
