Can I attach g-1145 to my application if I mail it to Dallas Texas PO Box ?

Hi everybody! I am about to send my package and I wanted to attach form G-1145 to know when they receive it. But when I went to read the instructions for g 1145 it says I can use it only if I send my package to Lewisville, Texas; Chicago, Illinois; and Phoenix, Arizona. But I was going to use usps priority mail so I would have to send it to PO Box in Dallas Texas. So now I’m confused if I can still use the form g 1145 because I would really like to. Does anyone know ?

You wanna read the the second line on G-1145 form that will clear your confusion.

Dallas Lockbox is a place, PO Box and Lewisville TX street are its mailing addresses. So yes, you can use it.
