My fiance received packet 3 and inside was a list of things to gather in preparation for the interview and a blank 1-134 form. Should that form be filled out and sent back to the embassy before the interview?

Unless noted otherwise, all is presented day of interview

You, the petitioner needs to fill that out and he needs to bring it to the interview.  Make sure you meet the income requirements and send your last filed tax return. Also make sure you get a co sponso if you are close to the boarder line with income.  Jamaica will require it in many cases
