I was curious to see if anyone knew of a civil surgeon in Boston area that would be able to provide a vaccination transcription?

Did you not complete all of your vaccinations at your overseas medical or was your DS3025 not marked in the "complete" box?

Thank you! That was actually super helpful and we were able to find a CS to do the transcription! I appreciate the help!!
